摘要
目的分析探讨超声与CT诊断新生儿颅内出血的临床价值。方法整群选择2014年1—12月之间来该院进行治疗的颅内出血的86例新生儿作为观察对象,同时应用超声与头颅CT对患儿进行诊断,分析对比两种检查方式的结果。结果 86例颅内出血的新生儿中,通过超声诊断确诊的有74例(脑实质出血:脑实质内有强回声团;脑室内出血:脑室强回声区;脉络丛血肿:脉络丛增宽),符合率为86.05%,有12例患儿被漏诊;应用CT扫描确诊的有80例(硬膜下出血:颅板下新月形高密度带;蛛网膜下腔出血:基底池高密度块影;脑室内出血:脑室内高密度铸型;脑实质出血:脑实质中有高密度块影),诊断符合率为93.02%,6例患儿被漏诊。收缩期最大流速高出正常值+2倍标准差,RI大于0.5为脑血管挛缩的诊断标准。结论超声与CT检查是诊断新生儿颅内出血的重要辅助手段,具有各自不同的优缺点。临床中应对高危新生儿进行常规超声检查,对于结果阴性的疑似病例还需要进行CT检查。
Objective To analyze and discuss the clinical value of ultrasound and CT in diagnosis of intracranial hemorrhage in newborns. Methods 86 cases of newborns with intracranial hemorrhage treated in our hospital from January 2014 to December were selected as the observation object, at the same time, the newborns were diagnosed with ultrasound and head CT, and the results of the two examination methods were compared and analyzed. Results In the 86 cases of newborns with intracranial hemorrhage, 74 cases were diagnosed with ultrasound(cerebral parenchymal hemorrhage: strong echoes in cerebral parenchyma, intra-cerebral ventricular hemorrhage: strong echoes in cerebral ventricle, choroid plexus hematoma;choroid plexus broadening), the accordance rate was 86.05%, and missed diagnosis occurred to 12 cases; 80 cases were diagnosed with CT scan(subdural hemorrhage: crescent high density zone under inner plate of skull, subarachnoid hemorrhage: high density mass shadow?of basal cistern, intra-cerebral ventricular hemorrhage: high density cast in cerebral ventricle, cerebral parenchymal hemorrhage: high density mass shadow in cerebral parenchyma), the diagnostic accordance rate was 93.02%, and the missed diagnosis occurred to 6 cases, the peak flow rate in the systolic period higher than normal value + 2 standard deviation and RI higher than 0.5 were the diagnostic standards of cerebrovascular contracture. Conclusion Ultrasound and CT examination were the important auxiliary means in diagnosis of intracranial hemorrhage in newborns,each method has its advantages and disadvantages, high-risk newborns should be given conventional ultrasound examination in clinic, and the suspected cases whose results were negative still needs CT examination.
