摘要
目的研究空心拉力螺钉内固定与克氏针张力带在髌骨骨折治疗中生物力学的对比。方法选取2013年2月~2015年2月我院接诊的60例髌骨骨折患者作为本次研究对象,按随机数字表法分为对照组和观察组,观察组采用空心拉力螺钉内固定,对照组采用克氏针张力带,观察两组患者膝屈曲角度及各参数受力、愈合情况、并发症比较。结果治疗后,观察组膝屈曲角度(45.10±2.78)α、髌腱拉力(2320.28±150.23)P、股四头肌拉力(2930.05±180.19)F、髌股关节作用力(2465.12±160.68)R均优于对照组(39.05±3.59)α、(1456.29±132.27)P、(1839.75±65.28)F、(1532.09±157.29)R差异具有统计学意义(P〈0.05);观察组住院时间(7.30±0.50)d、愈合时间(8.12±0.53)周均小于对照组(11.20±1.50)d、(9.50±0.78)周差异具有统计学意义(P〈0.05);观察组并发症总发生率10.00%(3/30)小于对照组40.00%(12/30),差异具有统计学意义(P〈0.05)。结论通过生物力学研究得出,空心拉力螺钉内固定治疗髌骨骨折有显著疗效,有利于患者术后早期功能锻炼,减少愈合时间。值得临床推广。
Objective To explore the differences of the biomechanical properties between cannulated lag screw internal fixation and Kirchner wire tension band in the treatment of patellar fracture. Methods 60 patients with patellar fracture who were in our hospital from February 2013 to February 2015 were evenly divided into observation group and control group at random, the observation group received the treatment of cannulated lag screw internal fixation, the control group were treated with Kirchner wire tension band. The knee flexion angles, force bearings, and incidences of complication were compared between the two groups. Results After operations, in the observation group, the knee flexion angles(45.10±2.78)α, the patellar tendon pulling forces(2320.28±150.23)P, the quadriceps femoris pulling forces(2930.05±180.19)F, and the patellofemoral joint reactive forces(2465.12±160.68)R were all superior than those of the control group, while in the control group, the parameters were(39.05±3.59)α,(1456.29±132.27)P,(1839.75±65.28)F, and(1532.09±157.29)R, the differences were statistically significant(P 0.05). The hospitalization time(7.30±0.50)d and the healing time(8.12±0.53)w of the observation group were both shorter than those of the control group's(11.20±1.50)d and(9.50±0.78)w, the differences were statistically significant(P 0.05). The total complication incidence of the observation group was 10.00%(3/30) that was lower than the control group's 40.00%(12/30), the difference was statistically significant(P 0.05). Conclusion It can be inferred from the biomechanical analysis that the treatment of cannulated lag screw internal fixation has an significant effect in the treatment of patellar fracture, which is conducive to the early postoperative functional exercises of patients, and reduce the healing time. It is worthy of clinical promotion.
