摘要
目的测定健康人群颈髓扩散张量成像(DTI)参数的正常值,分析其分布特征、变异及影响因素。资料与方法招募36例健康成年志愿者,行颈椎MR常规序列及颈髓矢状位DTI扫描,通过后处理工作站进行数据采集和影像重建,测定各个颈髓水平各向异性分数(FA)、表观扩散系数(ADC)值。通过扩散张量纤维束示踪(DTT)重建白质纤维束,观察纤维束走行及完整性。结果 DTT可清晰显示呈现头尾方向的颈髓内白质纤维束结构,且T2WI证实为正常颈髓的纤维束具有饱满、完整结构。全颈髓FA、ADC值与性别无关(P>0.05),但FA值与年龄呈显著负相关(r=-0.801,P<0.001),ADC值与年龄呈正相关(r=0.426,P<0.05)。不同颈髓水平的FA值、ADC值差异无统计学意义(P>0.05)。结论 DTI能够用于颈髓的定量分析,并可直观地显示白质纤维束结构,可作为常规MRI的重要补充。FA和ADC是DTI的敏感参数,用于定量分析时要注意年龄造成的影响。
Purpose To determine normative values of diffusion tensor imaging(DTI) parameters in healthy population, and to analyze their distribution, variations and influencing factors. Materials and Methods Conventional T1 WI, T2 WI and sagittal DTI scan of cervical spinal cord were conducted in 36 healthy adult volunteers. Through postprocessing workstation, regions of interest on sagittal views were placed in different levels of cervical spinal cord to measure fractional anisotropy(FA) and apparent diffusion coefficient(ADC) value. Then white matter fiber bundle was reconstructed with diffusion tensor tracking(DTT) for observation on its traveling and integrity. Results DTT could clearly show the white matter fiber bundle presented as rostral-caudal direction in cervical spinal cord. Normal spinal cord fibers confirmed by T2 WI images had full and complete structure. The whole cervical FA and ADC values showed no difference with gender(P〈0.05), but age had a significantly negative correlation with FA(r =-0.801, P〈0.001) while it was positively correlated with ADC(r =0.426, P〈0.05). There was no significant difference of FA and ADC values in various levels of cervical spinal cord(P〈0.05). Conclusion DTI can be used for quantitative analysis of cervical spinal cord and can visualize the structure of white matter fiber tracts so that it provides supplementery information for conventional MRI. Values of FA and ADC are sensitive DTI metrics, thus the variation of age needs to be considered in the quantitative analysis.
