先天性心脏病术后急性肾损伤患儿尿液NGAL、血清CysC水平变化及意义 被引量:14

摘要 目的探讨先天性心脏病(CHD)术后急性肾损伤(AKI)患儿尿液中性粒细胞明胶酶相关脂质蛋白(NGAL)和血清胱抑素C(Cys C)水平变化及意义。方法选择162例行心脏手术的CHD患儿,术后次日发生AKI44例(AKI组),未发生AKI 118例(非AKI组)。采用ELISA法检测两组术前及术后次日尿液NGAL和血清Cys C水平,并分析其与AKI的关系。结果 AKI组术后尿液NGAL、血清Cys C水平均高于术前及非AKI组(P均〈0.05);非AKI组手术前后尿液NGAL、血清Cys C水平比较差异均无统计学意义(P均〉0.05)。相关分析显示,术后尿NGAL(OR=2.314,95%CI为0.508~7.652)、血清Cys C(OR=1.401,95%CI为0.322~6.014)水平升高是发生AKI的独立危险因素(P均〈0.01)。结论 CHD患儿术后AKI发生率较高;AKI发生可能与术后尿液NGAL、血清Cys C水平升高有关。
