摘要
目的:总结施维群教授以厥阴法治疗肝硬化的经验。方法:通过从师学习,对施维群教授门诊、住院病历进行搜集、整理,总结其临证经验,探析施教授治疗肝硬化的中医辨证思路,并阐述其辨治的理、法、方、药思想。结果:施教授认为,肝硬化是各种慢性肝病发展的终末期,以肝功能衰减和门脉高压等为主要表现,其失代偿期常出现多种并发症,按仲景《伤寒论》厥阴法辨证可辨为类厥阴病、真厥阴病。临证当谨守病机,施以真厥阴法,即扶正温阳、养血化瘀;或类厥阴法,即寒热并调、虚实同治等。其辨证处方多以四逆汤、乌梅丸、麻黄升麻汤、三泻心汤等经方加减,其用经方治疗复杂肝硬化的临床疗效显著。结论:仲景的六经辨证法可执百病之牛耳,厥阴法治疗肝硬化只是其中之一的体现,经方治疗复杂的疾病不仅体现了《伤寒论》的价值,也启迪我们今后临证需体会和探究六经辨证的必要性和意义。
Objective： This article is aimed at summarizing Professor Shi＇s method of treating liver cirrhosis through Jue yin treatment.Method： Professor shi＇s experience is acquired by collecting and trimming the clinical and hospital records,as well as following the professor. This paper stated the theory,method and precriptions. Results： Professor shi argues that the cirrhosis is the end-stage of the development of various chronic liver diseases,being the main performance： portal hypertension and attenuating liver,and Cirrhosis often appears a variety of complications as decompensated. theory,It can be classified as（ class Jue yin） and the true Jue yin disease,according to the ＂ TCM＂ dialectical method; Method,the therapeutical？ principle of syndrome？ that the really Jue yin using Wenyang righting,blood circulation,and the class Jue yin using chills and fever,virtual and reality should be combined; Recipe： The classical rcecipe such as Si ni tang,ebony pills,ephedra cohosh soup,three Xie xin soups have made a significant effect in treating complex liver cirrhosis disease. Conclusion： Jue yin treatment of liver cirrhosis,a part of Zhang zhongjing＇s Six Classics dialectic,not only reflects the value of the ＂ Treatise on Febrile Diseases＂,but also inspires us to experience and explores the significance of philosophical dialectic.
