摘要
目的比较高频超声与MRI在急性闭合性跟腱断裂中的诊断价值。方法对69例急性闭合性跟腱损伤患者行超声及MRI检查,比较高频超声与MRI在急性闭合性跟腱断裂中的诊断结果。结果 69例患者经手术证实47例跟腱断裂(包括39例跟腱完全断裂,8例部分断裂),超声明确诊断38例(包括35例跟腱完全断裂,3例部分断裂),漏诊9例;MRI明确诊断45例(包括38例跟腱完全断裂,7例部分断裂),漏诊2例。超声诊断的灵敏度、特异度、阳性预测值、阴性预测值及准确度均显著低于MRI检查,差异具有统计学意义(P<0.05)。MRI诊断部分跟腱断裂符合率为87.5%,显著高于超声诊断的37.5%,差异有统计学意义(P<0.05)。结论高频超声由于操作简便、快捷(节约时间成本)、价格低廉、利于随访等优势,可作为跟腱断裂的首选检查方式,但术前明确诊断应通过MRI检查,尤其是对部分跟腱断裂患者。
Objective To compare the diagnostic value of high-frequency ultrasound and MRI in acute closed achilles tendon rupture.Methods 69 cases of patients with acute closed achilles tendon injury were implemented ultrasound and MRI examination. The diagnostic results of two methods in acute closed achilles tendon rupture were compared. Results Among 69 patients, 47 cases of achilles tendon rupture were confirmed by surgery, included 39 cases of complete achilles tendon rupture and 8 cases of partial rupture; 38 cases were confirmed by ultrasound（included 35 cases of complete achilles tendon rupture and 3 cases of partial rupture）, and 9 cases were misdiagnosed; 45 cases were confirmed by MRI（included 38 cases of complete achilles tendon rupture and 7 cases of partial rupture）, and 2cases were misdiagnosed. The sensitivity, specificity, positive predictive value, negative predictive value and accuracy rate of ultrasound were significantly lower than those of MRI examination, with statistical difference（P〈0.05）. The accordance rate of MRI in diagnosis of partial rupture was 87.5%, significantly higher than 37.5% of ultrasound, with statistical difference（P〈0.05）. Conclusions High-frequency ultrasound has the advantages of simple and quick operation, inexpensive price and convenient follow-up, which can be used as the preferred way for the diagnosis of achilles tendon rupture. But a definite preoperative diagnosis should be examined by MRI, especially for patients with partial achilles tendon rupture.
