卡维地洛对慢性心力衰竭患者心功能及血浆脑钠肽、N末端脑钠肽前体水平的影响 被引量:10

Impact of Carvedilol on Cardiac Function and Plasma Levels of BNP and NT-proBNP of Patients with Chronic Heart Failure

摘要 目的探讨卡维地洛对慢性心力衰竭(CHF)患者心功能及血浆脑钠肽(BNP)、N末端脑钠肽前体(NT-pro BNP)水平的影响。方法选取无锡市人民医院2012年6月—2014年6月收治的老年CHF患者120例,随机分为观察组和对照组,各60例。两组患者均给予常规治疗,观察组患者在常规治疗基础上给予卡维地洛治疗,两组患者均治疗6个月。比较两组患者治疗前后血压〔收缩压(SBP)和舒张压(DBP)〕、心功能指标〔左心室射血分数(LVEF)、左心室收缩末期内径(LVESD)和左心室舒张末期内径(LVEDD)〕及血浆BNP、NT-pro BNP水平。结果治疗前两组患者SBP、DBP、LVEF、LVEDD和LVESD比较,差异无统计学意义(P>0.05);治疗后观察组患者SBP和DBP低于对照组,LVEF高于对照组,LVEDD和LVESD小于对照组(P<0.05)。治疗前两组患者血浆BNP和NT-pro BNP水平比较,差异无统计学意义(P>0.05);治疗后观察组患者血浆BNP和NT-pro BNP水平低于对照组(P<0.05)。结论卡维地洛能有效改善CHF患者心功能,降低血浆BNP和NT-pro BNP水平,进而改善患者预后。 Objective To investigate the impact of carvedilol on cardiac function and plasma levels of BNP and NT-proBNP of patients with chronic heart failure. Methods A total of 120 patients with chronic heart failure were selected in the People's Hospital of Wuxi from June 2012 to June 2014, and they were randomly divided into control group and observation group, each of 60 cases. Patients of both groups received conventional treatment, and patients of observation group received extra carvedilol, both groups treated for 6 months. SBP, DBP, LVEF, LVESD, LVEDD, plasma levels of BNP and NT-proBNP were compared between the two groups before and after treatment. Results No statistically significant differences of SBP, DBP, LVEF, LVEDD or LVESD was found between the two groups before treatment (P 〉 0. 05); after treatment, SBP, DBP, LVEDD and LVESD of observation group were statistically significantly lower than those of control group, while LVEF of observation group was statistically significantly higher than that of control group ( P 〈 0. 05 ). No statistically significant differences of plasma level of BNP or NT-proBNP was found between the two group before treatment (P 〉 0.05 ), while plasma levels of BNP and NT-proBNP of observation group were statistically significantly lower than those of control group ( P 〈 0. 05 ). Conclusion Carvedilol can effectively improve the cardiac function and reduce the plasma levels of BNP and NT-proBNP of patients with chronic heart failure, is helpful to improve the prognosis.
