摘要
[目的]探讨七叶皂苷钠药物浓度及输液速度与兔耳缘输液静脉血管之间的关系,观察兔耳静脉炎的发生情况。[方法]以七叶皂苷钠药物浓度和输液速度作为研究因素,用0.9%氯化钠将4mg七叶皂苷钠稀释成不同浓度组(0.4mg/mL、0.13mg/mL),并以生理盐水作为对照,按不同输液速度(80gtt/min、50gtt/min、30gtt/min),以54只大耳白兔作为实验对象,采用析因设计方法将研究因素组合成9组实验组进行前瞻性随机对照研究,给药12h后采用美国静脉输液护士协会(INS)制定的静脉炎分级标准进行评价。[结果]生理盐水不同输液速度组均未发生静脉炎,给药组均有静脉炎发生;七叶皂苷钠输液速度对兔耳缘静脉炎的发生率和表现程度的差异无统计学意义(P>0.05),但浓度对静脉炎的发生率差异有统计学意义(P<0.05),输液速度和浓度对静脉炎的发生率存在交互作用(P<0.05)。[结论]七叶皂苷钠药物自身作用是导致静脉炎发生的主要原因;七叶皂苷钠输液速度对静脉炎发生率和表现程度均无影响,浓度的单独作用及浓度与输液速度的交互作用对静脉炎的发生率有影响,提示护理人员可采取维持正常或适当加快输液速度的措施,以降低七叶皂苷钠所致静脉炎的发生率。
Objective:To probe into the relationship between the concentration and transfusion speed of the sodi-um aescinate and the ears venous blood vessels in the rabbit and observe the occurrence of ears phlebitis of rab-bit.Methods:The concentration and infusion rate of sodium aescinate were taken as the study factors,4 mg sodi-um aescinate was diluted into different concentrations(0.4 mg/mL,0.13 mg/mL)with 0.9% sodium chloride, and the normal saline was taken as the control group.At different infusion rate(80 gtt/min,50 gtt/min,30 gtt/min), 54 big ear rabbits were taken as the experimental objects,factorial design method was used for formation of 9 experimental groups with study factors for prospective randomized controlled study.After administration of 1 2 h,the obj ects were evaluated with phlebitis grading standards made by INS.Results:There was no phlebitis oc-curred in obj ects in normal saline groups with different infusion rate,but there was phlebitis occurred in admin-istration group.There was no statistically significant difference in the influence of different infusion rate with sodium aescinate on the incidence and the expression level of ears phlebitis of rabbit(P〉0.05).there was statis-tically significant difference in the influence of the concentration on incidence of The difference was statistically significant(P〈0.05),but there was a significant difference in the incidence of ears phlebitis(P〈0.05).Conclu-sion:The main reason of the occurrence of phlebitis was the self effect of sodium aescinate.The infusion rate of sodium aescinate had no effect on the incidence and manifestations of phlebitis,concentration alone and interac-tion between concentration and infusion rate had effect for the incidence of phlebitis which suggested that the nursing staff can take measures to maintain normal infusion rate or speed up,so as to reduce the incidence of phlebitis induced by sodium aescinate.
