摘要
目的探讨脂联素基因多态性与2型糖尿病(T2DM)腰围、体重指数(BMI)的关系。方法选取218例T2DM患者,按照腰围和BMI的诊断标准分为单纯T2DM组和非单纯T2DM组,另选取健康正常80例为对照组,三组患者分别检测糖化血红蛋白(HbA1c)、腰围、总胆固醇(TC)、甘油三酯(TG)、脂联素。采用UNIQ-10柱提取外周全血中基因组DNA扩增目的基因片段,通过聚合酶链反应-限制性酶切片段长度多态性技术判断基因型,对脂联素基因+45位点和+331位点进行基因型和基因频率分析。结果非单纯T2DM组和单纯T2DM组的HbA1c、BMI、腰围、TC、TG和脂联素均明显高于对照组,差异均有统计学意义(t分别=5.87、4.12、8.68、4.31、6.77、10.86;4.69、3.23、4.79、4.01、6.06、9.23,P均〈0.05);非单纯T2DM组的HbA1c、BMI、腰围、TC和TG明显高于单纯T2DM组,差异均有统计学意义(t分别=4.31、4.36、7.69、4.15、6.35,P均〈0.05),非单纯T2DM组与单纯T2DM组的脂联素水平比较,差异无统计学意义(t=1.97,P〉0.05)。基因检测发现apM1Exon2+45基因位点存在多态性,有TT型、TG型和GG型。单纯T2DM组和非单纯T2DM组TT基因型的频率和T等位基因频率明显高于对照组,差异均有统计学意义(χ^2=5.13、6.57、7.12、5.12,P均〈0.05),单纯T2DM组和非单纯T2DM组之间TT基因型的频率和T等位基因频率差异无统计学意义(χ^2=1.85、2.01,P均〉0.05)。非单纯T2DM组患者的腰围和BMI水平在apM1Exon2+45位点的TT、TG和GG基因型中差异均无统计学意义(t分别=1.56、1.69、1.89,P均〉0.05)。基因检测发现apM1Exon2+331基因位点不存在多态性,全部是TT基因型。结论我国T2DM中脂联素基因+331位点不存在多态性,脂联素基因+45位点多态性与T2DM的发生有关,但与该人群腰围和BMI指标异常没有明显相关性。
Objective To explore the relationship between adiponectin gene polymorphism and waistline and BMI in pa-tients with type 2 diabetes(T2DM). Methods A total of 218 cases of T2DM were divided into simple T2DM group and non-simple T2DM group according to the diagnostic criteria of WC and BMI. Other healthy 80 cases as for the control group were selected as well. The HbA1c, waistline, TC, TG and adiponectin among three groups were detected. UNIQ-10 column was used to extract peripheral blood genomic DNA gene fragments amplified. The genotype judged by polymerase chain reaction-restriction fragment length polymorphism. Adiponectin gene locus +45 and +331 locus genotype and gene frequency were analyzed. Results HbA1c, BMI, waist circumference, TC, TG and adiponectin of non-simple T2DM group and simple T2DM group were significantly higher than those of control group(t=5.87, 4.12, 8.68, 4.31, 6.77, 10.86,4.69, 3.23, 4.79, 4.01, 6.06, 9.23, P 0.05). HbA1c, BMI, waist circumference, TC and TG of non-simple T2DM group significantly higher than those of simple T2DM group(t=4.31, 4.36, 7.69, 4.15, 6.35, P〈0.05). The adiponectin between non- simple and simple T2DM group was not statistically different(t=1.97, P〈0.05). Genetic testing found that apM1Exon2+45 genetic locus had polymorphism, including TT, TG and GG. TT genotype frequency and T allele frequency of simple T2DM group and non-simple T2DM group were significantly higher than the control group(χ~2=5.13,6.57,7.12,5.12, P〈0.05) while there was no statistical difference between non-simple and simple T2DM group(χ^2=1.85, 2.01, P〈0.05). In non-simple T2DM group, the waistline and BMI of apM1Exon2+45 sites TT,TG and GG genotype was not significantly different(t=1.56, 1.69, 1.89, P〉0.05). Genetic testing found that apM1Exon2+331 genetic locus did not exist polymorphism that is TT genotype. Conclusions In T2DM patients in China, adiponectin gene +331 sites do not exist polymorphism. The polymorphism of adiponectin gene +45 locus is related to T2DM, but is not related to waistline and BMI.
