摘要
目的通过序贯法测定盐酸羟考酮用于神经外科微创手术抑制瑞芬太尼痛觉过敏的半数有效剂量(medianeffective dose, ED_(50))。方法选择2017年1~5月清华大学玉泉医院在全身麻醉下行经乙状窦入路显微血管减压术患者20例,美国麻醉医师协会(American Society of Anesthesiologist, ASA)分级Ⅰ或Ⅱ级,年龄18~64岁,在开始缝合硬脑膜时给予盐酸羟考酮,根据序贯法原则设定浓度,当出现第7个交叉点时终止试验。术后10 min时患者镇痛口述分级评分(verbalrating score, VRS)1分为阴性。结果盐酸羟考酮用于神经外科微创手术抑制瑞芬太尼痛觉过敏的ED_(50),Dixon-Mood法计算为0.052 mg/kg(95%CI 0.049~0.053 mg/kg),Probit回归模型计算为0.052 mg/kg(95%CI 0.013~0.109 mg/kg)。结论盐酸羟考酮用于神经外科微创手术抑制瑞芬太尼痛觉过敏的ED_(50)为0.052 mg/kg。
Objective To determine the half of the effective dose(ED50) in the treatment of remifentanil hyperalgesia by minimally invasive neurosurgery via sequential method. Methods Twenty patients who were treated with sigmoid sinus microvascular decompression under general anesthesia were selected. These patients were classified by the American Society of Anesthesiologists(ASA) as Grade Ⅰor Ⅱ, aged 18-64 years. Intravenous oxycodone hydrochloride was administrated during the surgery according to the principle of sequential method, and the experiment was terminated when the seventh crossing point appeared. At 10 minutes after operation, the patient’s oral-reported analgesia score 1 was indicated as negative. Results A total of 20 patients were included in the statistics, oxycodone hydrochloride was used in neurosurgery to inhibit remifentanil hyperalgesia, ED50 was calculated as 0.052 mg/kg(95%CI 0.049-0.052) by Dixon-Mood method, while 0.052 mg/kg(95%CI0.013-0.109) by Probit regression model. Conclusions Half of the effective dose(ED50) of oxycodone hydrochloride used in minimally invasive neurosurgery for suppressing remifentanil hyperalgesia was 0.052 mg/kg.
