低温等离子消融术治疗儿童OSAHS的疗效观察 被引量:6

摘要 目的探讨低温等离子消融术在儿童阻塞性睡眠呼吸暂停低通气综合征(OSAHS)中的临床应用。方法分析2012年12月~2014年3月在我院接受治疗的OSAHS患儿的临床资料。入组患儿行扁桃体(伴或不伴腺样体)低温等离子消融术,评价其治疗效果并进行随访。结果本研究共纳入患儿60例,其中男33例,女27例。平均年龄为4.9±3.2周岁。治疗有效率为90.0%,术后患儿未出现呼吸、吞咽困难等严重并发症,大部分伴有短时间出现鼻阻塞症状,对症处理后好转。治疗后患儿的AHI(t=19.01,P〈0.01)和打鼾次数(t=29.86,P〈0.01)均显著降低,而LSpO_2(t=10.87,P〈0.01)显著升高,差异均有统计学意义。治疗前后患儿的炎性指标CRP、ESR、TNF-α和IL-6均未有统计学差异(P均〉0.05)。结论低温等离子消融术在儿童OSAHS中有满意的治疗效果。 Objective To analyze the clinical application of temperature-controlled coblation plasma surgery in children with obstructive sleep apnea hypopnea syndrome (OSAHS). Methods Clinical data of children with OSAHS received treatments at our hospital from 2012 to 2014 were retrospectively analyzed. Tonsillectomy (with or without adenoidectomy) were performed by temperature-controlled coblation plasma surgery. The curative effect was evaluated and the children were been follow-up. Results A total of 60 children were retrospective analyzed, including 33 boys and 27 girls. The average age was (4.9 ±3.2) years old. The treatment efficiency was 90.00. There was no seIious complication such as expiratory dyspnea and dysphagia happening after treatment. Most children accompanied with the symptom of rhinostegnosis shortly and became better after the symptomatic treatment. Both the AHI and the snoring times were obviously decreased after the treatment, but the LSpO2 was obviously increased. The differences had statistical significance. There was no statistical difference in inflammatory indexes including CRP, ESR, TNF-α and IL-6 before and after treatment. Conclusion The temperature-controlled coblation plasma surgery had the satisfactory therapeutic effect in treating the children with OSAHS.
