应用血栓弹力图预测短暂性脑缺血发作患者进展为脑梗死的临床研究 被引量:20

摘要 目的通过对短暂性脑缺血发作(transient ischemic attack,TIA)患者进行血栓弹力图(thromboelastography,TEG)分析,探讨TEG参数与TIA后进展为脑梗死的相关性,评价其预测价值。方法选择首次诊断TIA患者114例,行TEG检查,检测反应时间、凝固时间、凝固角、最大凝固振幅等参数指标。随访3个月,根据随访结果分为无脑梗死组78例和脑梗死组36例。比较2组参数并分析其与TIA后短期发生脑梗死的相关性。结果脑梗死组反应时间[(4.58±0.91)min vs(5.03±0.86)min,P=0.007]、凝固时间[(1.61±0.52)min vs(1.86±0.54)min,P=0.017]明显低于无脑梗死组,凝固角[(73.96±3.37)°vs(71.99±6.18)°,P=0.037]、最大振幅[(62.90±5.20)mmvs(60.09±6.39)mm,P=0.011]明显高于无脑梗死组。相关性分析显示,反应时间、凝固时间与脑梗死发生呈负相关(r=-0.418,r=-0.314,P<0.01);凝固角和最大振幅与脑梗死发生呈正相关(r=0.188,r=0.361,P<0.05)。logistic回归分析显示,反应时间、凝固时间越大,最大振幅越小,脑梗死发生风险越小。结论 TEG参数对TIA后短期进展为脑梗死风险有重要预测价值。 Objective To study the association between TEG parameters and ischemic stroke after TIA.Methods One hundred and fourteen TIA patients who underwent TEG were included in this study.The TEG parameters(reaction time,clotting time,clotting angle and maximal clotting amplitude)were assayed.The patients were divided into ischemic stroke group(n=36)and non ischemic stroke group(n=78)after they were followed up for 3months.The association between TEG parameters and ischemic stroke after TIA was analyzed.Results The reaction time and clotting time were significantly shorter whereas the clotting angle and maximal clotting amplitude were significantly higher in ischemic stroke group than in non ischemic stroke group(4.58±0.91 min vs 5.03±0.86 min,P=0.007;1.61±0.52 min vs 1.86±0.54 min,P=0.017;73.96°±3.37°vs 71.99°±6.18°,P=0.037;62.90±5.20 mmvs 60.09±6.39 mm,P=0.011).Correlation analysis showed that reaction time,clotting time,clotting angle were negatively associated with ischemic stroke whereas clotting angle and maximal clotting amplitude were positively associated with ischemic stroke(r=-0.418,r=-0.314,P0.01;r=0.188,r=0.361,P0.05).Logistic regression analysis showed that the longer the reaction time and clotting time were,the smaller the maximal clotting amplitude was and the lower the risk to develop ischemic stroke was.Conclusion TEG parameters play an important role in predicting ischemic stroke after TIA.
