摘要
目的:评价《咳嗽中医诊疗指南》(ZYYXH/T4-2008,以下简称《指南》)在不同行政级别的中医医院应用的适用性,为《指南》修订提供建议。方法:采用调查问卷方法,评价不同行政级别医院中《指南》的使用情况、质量水平、临床适用情况等。采用统计学基本描述,构建不同比较组,进行交叉或分层统计学分析。结果:《指南》平均使用率为75.2%,县级医院使用率低于市级医院;《指南》质量与水平、适用情况整体评分较高,各项得分均超过5分,但不同级别医院医师适用性评价结果有所不同,其中,"治则是否合理""与本单位专家经验比较""与重点专科诊疗水平比较""与相关标准的配套性""规范医疗服务的作用"等部分条目的评价差异具有统计学意义(P<0.05)。46.9%的医师建议及时更新《指南》,县级医院与市级医院在"指南评价的建议"与"指南最需要改进的地方"两个方面的评价存在统计学差异(P<0.05)。结论:《指南》质量水平较高,具有较好的临床适用性,临床使用率较高;不同级别中医院医师评价结果部分存在差异,是否使用过《指南》明显影响评价结果,使用《指南》的医师对其评价更高;建议及时更新《指南》,制定有针对性的指南实施推广策略,促进《指南》的推广应用。
Objective: To evaluate the applicability of Traditional Chinese Medicine Clinical Practice Guidelines(Cough)(ZYYXH/T4-2008) applied in different levels of Chinese medicine hospital in Shandong province and provided suggestions for revised guidelines. Methods: Using the questionnaire method to evaluate the utilization, quality and suitability of guideline in different administrative levels Chinese medicine hospitals. Adopting basic statistical description, comparison by construct different groups, or cross-stratified statistical analysis for data statistics. Results: The average utilization rate of this guide was 75.2%, and the utilization rate in the county hospitals was lower than in the city hospitals; Evaluation scores of quality and suitability of guideline were more than 5 points, but several scores are statistically different in different levels hospitals, such as ‘therapeutic principles, comparisons with the expertise within the unit', ‘comparisons with the diagnosis and treatment in key specialist', ‘supporting with related standards', ‘normative effect to medical service', etc(P<0.05). 46.9% of participants recommended update the guidelines, and evaluations between county hospitals and city hospitals were statistically different about ‘suggestion to guideline evaluation' and ‘the most needed improvement of the guideline'(P<0.05). Conclusion: Traditional Chinese Medicine Clinical Practice Guidelines(Cough) has good quality, which is using in the clinical practice with high utilization rate. Evaluation scores are different in different levels hospitals, whether to use the guidelines significantly affect the results of the evaluation, and guideline-user' evaluation scores are higher than guideline-nonuser. It is proposed to update the guidelines, develop targeted promotion strategy for implementation, promote the popularization and application of guidelines.
