三种方法测定糖化血红蛋白的一致性和可靠性分析 被引量:2

摘要 目的 探讨微粒色谱法、亲和层析法与高效液相色谱(HPLC)法测定糖化血红蛋白(GHb)的一致性和可靠性。方法 选取2014年6~9月在解放军总医院第一附属医院门诊及住院治疗的137例糖尿病患者为研究对象,空腹抽取双份全血标本。利用微粒色谱法、亲和层析法及HPLC法分别测定糖化血红蛋白A1(Hb A1c),每份标本测定2次。将入组的137例糖尿病患者参照HPLC法测定的Hb A1c为金标准分为3组:第1组Hb A1c含量≤6.5%;第2组6.5%0.05);回归分析显示,微粒色谱法与HPLC法所测的Hb A1c值呈良好的线性关系,Y=1.006 X+0.213,相关指数r2=0.885;层析法与HPLC法测定结果亦呈良好的线性关系,Y=0.923 X+0.338,相关指数r2=0.917;对于不同的糖尿病人群,3种方法检测结果亦呈现良好的相关性。结论 微粒色谱法和亲和层析法均能为临床提供与HPLC法所测糖化血红蛋白偏差较小的可靠数据,检验结果可被临床接受。微粒色谱法操作简单、快速,可为临床快速诊断提供方便。 Objective To discuss the consistency and reliability of Glycated hemoglobin determined by particles of chromatography, affinity chromatography and high performance liquid chromatography (HPLC) method. Methods 137 diabetics received ambulatory treatment or hospitalized in the First Affiliated Hospital of PLA General Hospital from June 2014 to September 2014 were selected as research objects, they were drew fasting double whole blood sample. Particles of chromatography, affinity chromatography and HPLC method was separately used to determine glycated hemoglobin A1 (HbA,o). Each specimen was measured twice. Taking HbAlo test by HPLC as gold standard, 137 diabetics were divided into three groups: group one (HbAlo level ~〈 6.5%), group two (6.5% 〈 HbAlc level 〈 7.5%) and group three (HbAlc level ≥ 7.5%). Whether there was statistical difference among three groups was analysed. Taking HbA1c test by Bio-Rad D10 HPLC method as target system, particles of chromatography and affinity chromatography as testing systems, regression equation was established, the degree of deviation between the results of target system and testing system was analysed. Results There was no significant difference in results of HbAlc among 3 methods (P 〉 0.05). Re- gression analysis showed that two methods had a good linear relationship between particles of Chromatography and HPLC method, Y = 1.006 X + 0.213, r2 = 0.885; There was also a good linear relationship between affinity chromatog- raphy and HPLC method, Y = 0.923 X + 0.338, r2 = 0.917. The results of three methods showed good correlation to different diabetics. Conclusion Both particles of chromatography and affinity chromatography can offer reliable data of small deviation for clinic, compared to HPLC method. The results can be accepted by clinic. Particles of chromatography is simple and fast to operate, which can provide convenience for clinical rapid diagnosis.
