摘要
目的研究高血压所造成的不同左室构型右心室功能的变化。方法选取2012年2月-2014年10月在我院确诊的原发性高血压患者86例和正常体检者30例作为研究对象,采用超声心动图方法对原发性高血压不同左室构型（Ⅰ正常构型、Ⅱ向心性重构、Ⅲ向心性肥厚、Ⅳ离心性肥厚）患者进行测定,分别测定左心室结构参数和右心室功能参数。结果Ⅲ组IVST、和LVPWT明显厚于其他各组（P〈0.05）;Ⅲ组及Ⅳ组LVMI明显增加（P〈0.05）。高血压不同左室构型各组右室E/A比值与Ea/Aa比值均降低（P〈0.05）;Ⅳ组别的右心室Sa和EF值明显低于对照组的数值（P〈0.05）。Ⅱ、Ⅲ、Ⅳ组脉冲多普勒和组织多普勒测算右心室Tei指数从Ⅱ组到Ⅳ组有逐渐递增的趋势。结论不同的左心室构型会对右心室功能造成损害;且脉冲多普勒和组织多普勒测算的Tei指数对于右心功能的变化都能够给予有效的评估。
Objectives To investigate the changes of right ventricular function in hypertension patients with different left ventricular patterns.Methods A total of 86 patients with primary hypertension diagnosed in our hospital from Feb2012 to Oct 2014 and 30 healthy volunteers were selected and ultrasonic cardiogram was conducted to detect the different left ventricular patterns（Ⅰ normal pattern,Ⅱ concentric remodeling,Ⅲ concentric hypertrophy,and Ⅳ eccentric hypertrophy） by measuring the left ventricular structure parameters and right ventricular function parameters.Results The IVST and LVPWT in group Ⅲ were evidently thicker than those in other groups（P〈0.05）;and LVMI in group Ⅲ and group Ⅳ was obviously increased（P〈0.05）.The right ventricle E/A ratio and Ea/Aa value was both reduced in groups of hypertension patients with different left ventricular patterns（P〈0.05）;and the right ventricle Sa and EF values in group Ⅳ were evidently lower than those of the control group（P〈0.05）.The right ventricle Tei index of group II,IU and IV were measured by pulse doppler and tissue doppler and it showed an increasing tendency from group Ⅱto Ⅳ.Conclusion Different left ventricular patterns can damage the right ventricular function.Tei index measured by doppler pulse and tissue doppler can provide effective assessment on changes of right ventricular function.
