摘要
目的:探讨经阴道三维及彩色多普勒超声诊断子宫切口妊娠的临床价值.方法:选取2014年3月-2014年12月某院收治的32例子宫切口妊娠患者为观察组,再选取同期孕囊着床位置于子宫底部或子宫体的37例患者为对照组,两组患者均使用经阴道三维及彩色多普勒超声检查,比较两组超声图像特征.结果:观察组孕囊与切口距离、切口肌层厚度均小于对照组,具有统计学差异(P 〈0.05);观察组宫腔内有异常回声率低于对照组,具有统计学差异(P 〈0.05);观察组子宫切口假腔率低高于对照组,具有统计学差异(P 〈0.05);此外,观察组的切口肌层血流信号较对照组丰富,具有统计学差异(P 〈0.05).结论:经阴道三维及彩色多普勒超声可观察切口肌层厚度、血流信号、孕囊种植部位、子宫切口处前壁的完整性,可作为判断子宫切口妊娠的依据,临床制定治疗方案提供参考.
Objective:To investigate the clinical value of vaginal 3Dand color Doppler ultrasonography to the diagnosis of CSP.Methods:Select 32 patients with CSP received in some hospital during the period from March 2014 to December 2014 as the observation group,select another concurrent 37 patients whose gestation sac implanted on the fundus of uterine or on corpus uteri,all the patients in both group were examined with vaginal 3Dand color Doppler ultrasonography,observe the features of the ultrasonic images.Results:In observation group,the distance between gestation sac and incision was shorter and thickness of muscular coat was thicker than that in control group,and the differences were statistically significant(P<0.05).The occurrence rate of abnormal echo in uterine was less than that in control group,which was statistically significant(P<0.05),the rate of false lumen of uterine incision in observation group was higher than that in control group,which was statistically significant(P<0.05),besides,the flow signals around the fundus of incision was more than that in control group,which was also statistically significant(P<0.05).Conclusion:Vaginal 3Dand color Doppler ultrasonography can be applied to detect the thickness of fundus of incision,flow signals,implantation site of gestation sac,the integrity of paries anterior of cesarean scar.And it can be the judging base for CSP and provide reference for the determination of clinical treatment protocol.
