摘要
目的针对乌鲁木齐市三级医院与社区卫生服务中心以双向转诊制度为纽带开展互动合作情况进行研究,为完善两级医疗机构间的互动合作提供理论支撑。方法采用问卷调查法、专家访谈法对3家三级医院和13家社区卫生服务中心的医务人员和管理者进行调查和访谈,并对数据进行统计分析。结果三级医院90.66%的医生认为和社区卫生服务中心互动合作主要障碍是合作渠道不通畅,其次为缺乏合作意识,占85.81%,第三是缺乏相关政策支撑;在社区卫生服务中心,社区医生除了上转患者外,最希望与三级医院开展互动的内容排在前三位的是开展专业技术指导、开展远程医疗、共享图书信息资源,分别占96.69%、94.58%、88.25%。结论乌鲁木齐市三级医院与社区卫生服务开展以双向转诊为纽带的互动合作,但也存在着如政府缺位、医保政策体系不完善、互动网络化建设尚待改进、缺乏互动管理监督机制等影响因素,进而提出从加强政府支持、转变社区居民的就医意识、医疗保险制度介入、快推进分级诊疗及信息化与评价体系建设,积极组建医疗联合体,积极推进医师多点执业等六个方面的意见与建议。
Objective To study interact cooperation for two-way referral system of tertiary hospitals and community health service centers in Urumqi, this paper aims to improve interaction and cooperation between the two levels of medical institutions to provide theoretical support. Methods Through questionnaires and expert interview to investigate and interview the medical staffs and managers of 3 tertiary hospitals and 13 community health service centers. And then, the data was statistically analyzed. Results It demonstrated that 90.66% of doctors of tertiary hospitals believe that the major obstacle of interaction between tertiary hospitals and community health service centers is lack of cooperation, followed by lack of cooperation consciousness, accounting for 85.81 percent, and the third is the lack of policy support, in community health service centers, community physicians in addition to the transfer of patients, the most hope and tertiary hospitals interactive content is to carry out the top three professional and technical guidance, to carry out telemedicine, shared library information resources, accounting for 96.69%, respectively, 94.58%, 88.25%. Conclusion Tertiary hospitals and community health service to carry out two-way interaction and cooperation referral as a link, but there are also such as absence of government, health insurance policy system is imperfect, interactive network construction to be desired, the lack of interaction management and supervision mechanism, influence factor, and then put forward to support from the Government to strengthen, change medical awareness of community residents,medical insurance system intervention, promoting fast diagnosis and information classification and evaluation system,and actively established medical association, and actively promote the multi-physician practice in six areas comments and suggestions.
