摘要
目的经阴道三维超声测量孕期宫颈预测早产的临床应用价值。方法2012年6月～2014年6月在本院建卡产前检查符合纳入标准的孕妇200例，追踪妊娠结局，其中早产组15例，足月产组185例。两组孕妇均在第22、27、31、35周采用经阴道三维超声检测宫颈结构。结果早产组宫颈长度均值（2．76±0．67）cm，明显短于足月产组（3．69±0．63）cm，差异有统计学意义（P〈0．05）；早产组宫颈指数均值〉0．5，足月产组宫颈指数均值〈0．5，差异有统计学意义（P〈0．05）；早产组宫颈漏斗面积均值（3．41±1．03）cm2，足月产组宫颈漏斗面积均值（1．26±0．42）cm2，差异有统计学意义（P〈0．05）。结论早产孕妇的宫颈长度会缩短，宫颈指数、宫颈漏斗面积、宫颈漏斗宽度偏大，经阴道三维超声评估妊娠期孕妇宫颈结构，是预测早产发生的有效手段。
Objective To investigate the clinical application value of transvaginal three-dimensional ultrasound mea- surements of pregnancy cervix in predicting pregnancy. Methods A total of 200 cases of pregnant women who had cards of prenatal examination and met the inclusion criteria in our hospital from June 2012 to June 2014 were chosen and their pregnancy outcomes were tracked. There were 15 cases in the pretermgroup, and 185 cases in the full-term group. The cervical structures of pregnant women in the two groups were detected at the 22nd, 27th, 31st and 35th week by transvaginal three-dimensional ultrasound. Results Mean cervical length in the preterm group[（2.76±0.67） cm], was significantly shorter than that in the full-term group [（3.69±0.63）cm], and the difference was statistically significant （P〈0.05）. The mean cervical index in the preterm group was more than 0.5, and that in the full-term group was less than 0.5. The difference was statistically significant （P〈0.05）. The mean cervical funnel area in the preterm group was [（3.41±1.03） cm2], and that in the full-term group was [（1.26±0.42） cm2]. The difference was statistically significant （P〈 0.05）. Conclusion The results show that the cervical length in preterm patients reduces, cervical index, cervical funnel area, and cervical funnel width are too large. Transvaginal three-dimensional ultrasound in assessing the cervix struc- ture of pregnant women is an effective means of preterm forecast.
