摘要
目的探讨葡萄糖酸钙在上消化道出血治疗中的临床疗效。方法选择上消化道出血患者136例的临床资料进行回顾性分析。其中研究组64例患者采用葡萄糖酸钙联合奥美拉唑治疗,对照组72例患者单纯给予奥美拉唑治疗;比较两组患者的临床疗效。结果研究组治疗后(31.8±6.9)h止血,对照组治疗后(47.4±5.2)h止血,两组比较,差异有高度统计学意义(P<0.01)。研究组总有效率显著高于对照组,差异有高度统计学意义(P<0.01)。研究组止血不彻底发生率、原发病灶再出血发生率均显著低于对照组(P<0.05);两组心前区不适、头痛等并发症比较差异无统计学意义(P>0.05)。治疗第3天,两组患者脉搏有显著下降,研究组下降更明显(P<0.01);收缩压有显著升高,研究组升高更明显(P<0.01)。治疗5 d后,患者血红蛋白与治疗前比较,均有显著回升,但研究组升高更明显(P<0.01)。结论葡萄糖酸钙和奥美拉唑治疗上消化道出血能够显著提高临床疗效,缩短止血时间,降低止血不彻底以及原发病灶再出血率。
Objective To discuss efficacy of calcium gluconate in treatment of upper gastrointestinal bleeding. Meth- ods Clinical data of 136 cases with upper gastrointestinal bleeding were respectively analyzed. 64 cases of study group were treated by calcium gluconate combined with Omeprazole, and 72 cases of control group were treated by Omepra zole. Clinical efficacy of two groups was compared. Results Stanch bleeding time of study group was （31.8±6.9）h; and control group was （47.4±5.2）h. There was significant difference between two groups（P〈0.01）. Total efficiency of study group was higher than control group（P〈0.01）. The incidence of hemostatic uncomplete and primary lesion rebleeding of study group was lower. The complications of precordial discomfort, headache of study group had no significant differ ence compared with control group（P〉0.05）. After treatment of 3 d, pulse rate of two groups were lower than the first day, and study group was lower than control group（P〈0.01 ）; After treatment of 3 d, SBP of two groups were lower than the first day, and study group was lower than control group（P〈0.01）. After treatment of 5 d, Hb of two groups were higher than before treatment, and study group was higher than control group（P〈0.01）. Conclusion Clinical efficacy is improved, stanch bleeding time is shorted, and hemostatic uncomplete and primary lesion rebleeding are decreased with calcium gluconate combined with Omeprazole in treatment of upper gastrointestinal bleeding.
