肾性骨病患者血清IGF-1、iPTH、BALP、OC含量检测及其与骨密度的关系研究 被引量:10

Detection of serum IGF-1, iPTH, BALP, OC levels in renal osteodystrophy patients and their correlations with bone mineral density

摘要 目的研究肾性骨病患者血清胰岛素样生长因子-1(insulin-like growth factor 1,IGF-1)、骨钙素(osteocalcin,OC)、全段甲状腺素(intact parathyroid hormone,i PTH)、骨碱性磷酸酶(bone alkaline phosphatase,BALP)的含量及其与骨密度的关系。方法 2013年4月至2014年5月,将在我院就诊的80例慢性肾脏病患者纳入研究的观察组,将80例健康者纳入研究的对照组,检测两组患者的血清IGF-1、i PTH、BALP、OC含量以及L3、L4椎体、股骨颈、Ward’s区、大转子的骨密度,分析骨密度水平与血清学指标的相关性。结果 (1)血清学指标:观察组患者的血清IGF-1水平(145.2±17.8)μg/L低于对照组(245.2±31.5)μg/L,观察组i PTH(234.8±27.3)ng/L、BALP(94.9±11.5)U/L、OC水平(13.9±1.6)μg/L均分别高于对照组(58.8±6.9)ng/L、(35.6±4.5)U/L、(6.7±0.8)μg/L(P<0.05);(2)骨密度水平:观察组患者的L3腰椎(0.817±0.104)g/cm2、L4腰椎(0.742±0.094)g/cm2、股骨颈(0.744±0.108)g/cm2、Ward’s三角区(0.762±0.091)g/cm2、大转子的骨密度水平(0.804±0.089)g/cm2均低于对照组(1.056±0.186)g/cm2、(1.008±0.13)g/cm2、(0.907±0.12)g/cm2、(0.899±0.104)g/cm2、(0.948±0.118)g/cm2(P<0.05);(3)血清IGF-1水平与骨密度呈正相关(P<0.05);i PTH、BALP、OC水平与骨密度呈负相关(P<0.05)。结论肾性骨病患者的骨密度显著下降;且骨密度与血清IGF-1、i PTH、BALP、OC含量密切相关,IGF-1水平高及i PTH、BALP、OC水平低,则骨密度高。 Objective To study the serum insulin-like growth factor 1 ( IGF-1 ), intact parathyroid hormone ( iPTH ), bone alkaline phosphatase ( BALP ), osteocalcin ( OC ) levels in renal osteodystrophy patients and their correlations with bone mineral density.Methods Eighty cases of chronic kidney disease patients in our hospital from from 2013 April - 2014 May were enrolled in the observation group. Eighty cases of the healthy were enrolled in the control group. Then serum IGF-1, iPTH, BALP, OC levels and bone density of L3, L4 vertebrae, trochanter, Ward’s region in the two groups were detected.Results ( 1 ) Serum index: IGF-1 level of observation group ( 145.2±17.8 ) μg / L was lower than control group ( 245.2±31.5 ) μg / L; iPTH level of observation group ( 234.8±27.3 ) ng / L; BALP level of observation group ( 94.9±11.5 ) U / L, OC level of observation group ( 13.9±1.6 ) μg / L was higher than control group ( 58.8±6. 9 ) ng / L, ( 35.6±4.5 ) U / L, ( 6.7±0.8 ) μg / L individually( P〈0.05 ). ( 2 ) Bone density: bone density of L3 vertebrae ( 0.817±0.104 ) g / cm2, L4 vertebrae ( 0.742±0.094 ) g / cm2, trochanter ( 0.744±0.108 ) g / cm2, Ward’s region ( 0.762±0.091 ) g / cm2, greater trochanter ( 0.804±0.089 ) g / cm2 of observation group were lower than those of control group ( 1.056±0.186 ) g / cm2, ( 1.008±0.13 ) g / cm2, ( 0.907±0.12 ) g / cm2, ( 0.899±0.104 ) g / cm2, ( 0.948±0.118 ) g / cm2 (P〈0.05 ); ( 3 ) Serum IGF-1 level was positively correlated with bone density (P〈0.05 ). Serum iPTH, BALP, OC levels was negatively correlated with bone density (P〈0.05 ).Conclusions Bone density of renal osteodystrophy patients decreases signiifcantly and is closely correlated with serum IGF-1, iPTH, BALP, OC levels. High level of IGF-1 and low levels of iPTH, BALP, OC demonstrate high bone density.
