螺旋CT引导下氩氦靶向治疗恶性实体肿瘤的疗效观察

摘要 目的:通过临床试验,观察螺旋CT引导下氩氦靶向治疗恶性实体肿瘤的疗效及其影响因素,为恶性实体肿瘤患者的临床治疗方案选择提供依据。方法:选择150例恶性实体肿瘤患者,在螺旋CT引导下进行氩氦刀冷冻消融治疗,分析瘤体冰球覆盖率,并在术后随访监测,记录疗效及并发症。结果:150例患者共有瘤体265个,瘤体直径范围为1.2-12 cm,中位数为4.21 cm。与直径〉3 cm的瘤体比较,瘤体≤3 cm的冰球覆盖率较好(P〈0.05)。术后1、3、6、12个月的肿瘤消融治疗有效率分别为81.13%、71.76%、71.07%、68.10%,肿瘤控制率分别为96.23%、91.37%、85.54%、80.95%;肿瘤直径和肿瘤分期是术后1年疗效的影响因素(P〈0.05)。结论:螺旋CT引导下氩氦靶向治疗恶性实体肿瘤的效果较好,可以在临床上应用。 Objective: To observe the clinical effect of spiral CT guidance argon-helium cryablation on malignant solid tumors and the influencing factors of the treatment effects. Methods: 150 patients of malignant solid tumors were selected who were confirmed by pathology. After the cryablation therapy, follow-up were carried out and the treatment effect and the related complications were recorded. Results: There were 265 tumors in the 150 patients. The diameter of tumors ranged from 1.2 cm to 12 cm, with a median of 4. 21cm. Compared with the ice coverage of tumors larger than 3 cm in diameter, ice coverage of tumors less than 3 cm was better(P 〈 0. 05 ). The efficient rates were 81.13%, 71.76%,71.07% and 68. 10% in 1,3,6,12 months respectively, and the controlling rates were 96. 23%,91.37%,85.54% and 80.95%, respectively. Tumor diameter and tumor staging were the influence factors of curative effect in one year after therapy ( P 〈 0. 05 ). Conclusion: Spiral CT guidance argonhelium cryablation was good for the treatment of malignant solid tumors, which could be used in clinics.
