摘要
目的 通过对健康体检者的肿瘤标志物甲胎蛋白(AFP)、癌胚抗原(CEA)、CA125和鳞状细胞癌抗原(SCCAg)检测结果的分析,评估肿瘤标志物参考范围设置的合理性.方法 回顾分析2013年1月至12月我院33512名健康体检人群AFP、CEA、CA125和SCCAg检测结果,按年龄、性别观察肿瘤标志物分布特点.结果 去除异常生化检测结果,共28934名健康体检者纳入研究.大于95%的体检者血清AFP、CEA、CA125和SCCAg处于正常参考范围内.异常结果阳性率分别是SCCAg 4.3%,CEA 2.3%、CA125 1.7%、AFP 0.2%.AFP、CEA、CA125、SCCAg不同浓度梯度、不同性别间比较,差异均有统计学意义(P<0.05).不同性别CEA阳性检出率均随着年龄增加逐渐增高,各年龄组间差异均有统计学意义(均P<0.05).SCCAg在30~ 60岁男性异常率较高.女性50岁以上体检者的CA125阳性检出率低于50岁以下者,差异有统计学意义(P<0.05).结论 实验室应根据自己的检测人群建立肿瘤标志物的参考范围.肿瘤标志物浓度受机体炎症、肝肾功能等影响,对体检人群的异常值应合理解释.
Objective To evaluation the rationality of the reference interval of alpha fetoprotein (AFP), carcinoembryonic antigen (CEA), CA125 and squamous cell carcinoma antigen (SCCAg), in the healthy examination. Methods From January to December in 2013, 33512 cases of healthy subjects were enrolled in this study. The results of serum levels of AFP, CEA, CA125 and SCCAg were classified analyzed by age and gender. Results After removal of abnormal biochemical test results, a total of 28934 healthy subjects were included in this study. The serum levels of AFP, CEA, CA125 and SCCAg in more than 95% subjects were in the normal reference range. The rate of abnormal results of SCCAg, CEA, CA125, AFP were 4.3% , 2.3% , 1.7% , O. 2% separately. The serum levels of AFP, CEA, CA125, and SCCAg in different level groups and different gender had statistically different ( P 〈 O. 05 ). The positive rate of CEA was increased with increase of ages in both genders ( P 〈 0.05 ). The abnormal rate of SCCAg in 30-60 age group in male was higher than others. The positive rate of CA125 in female over 50 years old was lower than those of below 50 years old(P 〈 0.05 ). Conclusion The clinical laboratories should set their own reference range of tumor markers based on regional population. The concentration of tumor markers is influenced by many factors like inflammation, liver and kidney function. When abnormal tumor markers are observed in healthy check subjects, reasonable explanation is needed.
