摘要
目的了解天津市社区老年人轻度认知功能障碍(mild cognitive impairment,MCI)患病率及人群分布特征。方法采用多阶段整群随机抽样方法,利用Petersen等诊断标准对天津市塘沽区60岁以上老年人进行筛查,对筛选出的MCI患者进行问卷调查,对其人口学特征进行Pearson检验和多因素非条件Logistic回归分析。结果MCI患病率为10.89%,MCI患病率在不同性别、年龄、经济收入、文化程度、婚姻状况的差异均有统计学意义(均有P〈0.05)。多因素Logistic回归分析结果显示:危险因素包括:高龄(OR=3.12,95%CI:1.46-6.55)、离婚(OR=1.61,95%CI:1.55-1.67),保护因素包括:文化程度(OR=0.16,95%CI:0.08-0.35)。结论天津市社区60岁以上老年人群MCI患病率较高(10.89%),高龄、低文化程度、离婚可能是MCI的高危因素,应及早采取有效措施,延缓患者病情进展。
Objective To investigate the prevalence and distribution of MCI and related socio-demographic factors among elderly population in Tianjin. Methods The multi-stage cluster random sampling method was used among elderly population( 60 years) in Tang Gu District,Tianjin for prevalence survey. The Petersen diagnosis standard was used to screen patients of mild cognitive impairment who were willing to cooperate with this study and accept the questionnaire survey. Pearson test and multiple-factor unconditioned Logistic regression methods were conducted to explore the demographic factors of MCI. Results The prevalence of MCI was 10. 89%. The prevalence of MCI in different gender,age,income,education level and marital status had significantly statistical difference( all P〈0. 05). Multivariate Logistic regression analysis showed that the risk factors included age( OR = 3. 12,95% CI: 1. 46-6. 55) and divorce( OR = 1. 61,95% CI:1. 55-1. 67). It also showed that protective factors included the cultural level( OR = 0. 16,95% CI: 0. 08-0. 35). Conclusions Prevalence of MCI( 10. 89%) was high among elderly population aged over 60 in Tianjin community. The aged,low educational level and divorce might be the risk factors of MCI,and effective measures should be taken imperatively to slow disease progress in patients.
