杭州市土源性线虫感染现况及防治效果评价 被引量:18

Infection status of soil-transmitted nematodes and evaluation of control effect in Hangzhou

摘要 目的了解杭州市土源性线虫感染现状,并与历年调查数据进行比较分析,对几十年的综合防治措施进行效果评价。方法收集3岁及以上且在杭州市居住5年及以上者的粪便样本,采用改良加藤厚涂片法检查蛔虫卵、钩虫卵及鞭虫卵,3周岁~12周岁儿童加做透明胶纸肛拭法检查蛲虫卵,并对检查结果进行统计分析。结果 2013年共检测4 133人,检出土源性线虫卵27人,总感染率为0.65%,其中蛔虫、钩虫、鞭虫虫卵的感染率分别为0.22%(9/4 133)、0.41%(17/4 133)、0.02%(1/4 133),未检出蛲虫卵。男性土源性线虫感染率明显高于女性,差异有统计学意义(χ2=13.134,P=0.000);山区感染率高于效区,差异有统计学意义(χ2=5.574,P=0.018)。感染人群以50岁以上的农民为主,占81.48%(22/27),小学以下文化程度者占70.37%(19/27)。与1988年的调查资料(72.62%)相比土源性线虫病的总感染率下降至2013年的0.65%。结论杭州市土源性线虫病的防治成效显著,钩虫病是杭州市目前主要的土源性线虫病,防治的重点人群是经济落后的农村老年人群。 Objective To learn infection status of soil-transmitted nematodes in Hangzhou,compare and analyze the survey data over the years and evaluate the effect of comprehensive prevention and control measures for decades. Methods Feces of the persons were collected,who was 3 years old and above and resided in Hangzhou city for five years and above. Kato-katz thick smear method was used to detect roundworm,hookworm,and whipworm eggs. For the persons of 3-12 years old,cellophane tape method was used to detect pinworm eggs. The results were analyzed statistically. Results A total of 4 133 people were detected,27 persons of who were detected soil borne nematode egg. The total infection rate was 0. 65%. The infection rate of roundworm,hookworm,and whipworm was 0. 22%(9 /4 133),0. 41%(17 /4 133),0. 02%( 1 /4 133) respectively. No pinworm was detected. Soil-transmitted nematodes' infection rate of male was significantly higher than that of women,the difference had statistical significance( χ2= 13. 134,P = 0. 000). The infection rate of mountain areas is significantly higher than that of suburban( χ2= 5. 574,P = 0. 018. Infected people was mostly farmers with over 50 years old,accounting for 81. 48%(22 /27). Primary school education accounted for 70. 37%(19 /27). Compared with the investigation data in 1988 year(72. 62%),infection rate of soil-transmitted nematodes dropped to 0. 65% in 2013. Conclusion The comprehensive prevention and control of soil-transmitted nematodes was effective significantly in Hangzhou city. Hookworm disease in Hangzhou city is currently the main soil-transmitted nematode disease. Focus groups for prevention is the rural elderly people of the economic backwardness.
