常规湿化与未湿化低流量吸氧的临床效果及不适反应比较 被引量:15

Clinical Effect and Adverse Reaction Comparison between Conventional Wet Low-flow Oxygen and Dried Low-flow Oxygen

摘要 目的以随机对照设计探讨常规湿化与未湿化低流量吸氧应用于呼吸系统患者的临床效果及不适反应的差异性。方法便利抽样选取2012年1月至2013年10月在呼吸内科长期低流量吸氧的肺部疾病患者434例,采用随机数字表法将受试对象分成两组,即常规使用湿化液组、不使用湿化液组。入选患者均给予低流量、低浓度吸氧,吸氧时间每日>6h,观察两组患者不适反应、缺氧改善情况、继发呼吸道感染、住院时间及累计吸氧时间,比较两者之间的差异性。结果两组患者鼻黏膜出血、鼻咽部干燥、吸氧异味及胸部不适症状发生率均无统计学差异(均P>0.05)、恶心症状未湿化组发生率高于对照组(P<0.05);两组患者缺氧症状改善情况比较差异无统计学意义(P>0.05),湿化组继发呼吸道感染发生率高于未湿化组;两组患者住院时间比较差异无统计学意义(P>0.05),湿化组累计吸氧时间、所消耗护理时间高于未湿化组,未湿化组患者满意度高于湿化组(P<0.05)。结论未湿化低流量吸氧相比湿化低流量吸氧,可以减少呼吸系统患者的吸氧时间及继发呼吸道感染的发生率,但可能造成患者恶心症状。在临床过程中,可以在改善患者恶心症状的同时提倡未湿化吸氧,以降低继发呼吸道感染率,减轻患者经济负担,减少护士的工作量。 Objective By using randomized controlled trial to discuss the clinical effects and differences of the adverse reaction between conventional wet low-flow oxygen and dried low-flow oxygen for patients with respiratory disease.Methods By convenience sampling,434 cases of patients with pulmonary disease were selected and randomly divided into wet low-flow oxygen group and dried low-flow oxygen group.All patients were had low-flow,low concentration oxygen,and the oxygen intake time were longer than 6hours per day.The adverse reaction,hypoxia situation improvement,secondary respiratory infections,hospitalization time and total oxygen intake time were compared between two groups of patients.Results The incidence rates of nasal mucosa bleeding,dry nasopharyngeal,peculiar smell of the oxygen and chest discomfort had no statistical significance between two groups(all P〉0.05),while the incidence rate of nausea in dried low-flow oxygen group was higher than control group(P〈0.05).There was no difference between the situation improvements of hypoxia between two groups(P〉0.05),while the incidence rate of secondary respiratory infections in wet low-flow oxygen was higher than dried low-flow oxygen group.There was also no difference of the hospitalization day between two group(P〉0.05),while the total oxygen intake time,nursing time in wet low-flow oxygen group were higher than dried low-flow oxygen group,and the patient's satisfaction level in dried low-flow oxygen group was higher than wet low-flow oxygen group(P〈0.05).Conclusion The dried low-flow oxygen can reduce the oxygen intake time and the incidence rate of secondary respiratory infections,but could also cause nausea.In clinical application,it can improve patient's nausea symptoms while using dried low-flow oxygen to reduce the secondary respiratory infections,reduce the financial burden on patients,and also reduce the workload of nurses.
