摘要
报告1例棘层松解性间变性乳房外Paget病。患者男,78岁。阴囊浸润性红斑伴糜烂渗出1年。皮损组织病理检查:表皮棘层松解,表皮内及毛囊上皮见散在或呈巢状的异形细胞。免疫组化:S-100蛋白(-)、HMB45(-)、melan-A(-)、癌胚抗原(CEA)(+++)、上皮膜抗原(EMA)(+)、CK7(+++)、CK20(-)、囊泡病液体蛋白(GCDFP)-15(+)和Ki-67(<10%)。特殊染色:PAS染色(部分肿瘤细胞阳性),阿辛蓝染色(部分肿瘤细胞阳性)。直接免疫荧光:阴性。诊断:棘层松解性间变性乳房外Paget病。
A case of acantholytic anaplastic extramammary Paget＇s disease is reported. A 78-year-old male presented with infiltrative erythema with erosion and exudation on his scrotum for 1 year. Histopathological examination demonstrated acantholysis, atypical cells scattered or distributed in nest pattern within the epidermis and follicular epithelium. Immunohistochemisty showed S-100 protein（-）, HMB45（-）, melan-A（-）, CEA（＋＋＋）, EMA（＋）, CK7（＋＋＋）, CK20（-）, GCDFP-15（＋）and Ki-67（10%）. PAS and Alcian blue staining both showed some tumor cells were positive. Direct immunofluorescence was negative. The diagnosis of acantholytic anaplastic extramammary Paget＇s disease was made.
