摘要
目的探讨快速康复外科理念在胸腹腔镜食管癌切除术患者围手术期护理中应用的效果。方法选取汕头市中心医院肿瘤外科2012年8月至2013年10月收治的接受胸腹腔镜食管癌切除术的患者97例，其中2013年3—10月接受手术患者48例（A组），围手术期采用快速康复外科理念进行处理，2012年8月至2013年3月接受手术患者49例（B组），围手术期按常规处理，比较2组术后引流量、术后排气时间、术后拔除胸腔引流管时间、术后应用镇痛药的次数、术后住院时间、住院总费用和术后并发症发生情况以及患者满意度。结果与B组相比，A组在术后排气时间、术后拔除胸腔引流管时间、术后应用镇痛药的次数、术后住院时间（t值分别为2．165、14．996、2．488、1．998）及患者满意度方面坼值为8．926）均优于对照组，差异有统计学意义；2组在术后引流量、住院总费用、术后并发症发生率上比较差异无统计学意义。结论基于快速康复外科理念的护理干预能促进胸腹腔镜食管癌切除术患者的快速康复，改善患者的临床结局及医患关系。
Objective The aim of the study was to explore the application of fast-track surgery con- cept in perioperative patients with esophageal cancer underwent thoracoscopic and laparoscopic esophage- tomy. Methods From August 2012 to October 2013, a total of 97 patients who were performed thoracoscopic and laparoscopic esophagectomy in Department of Oncology Surgery, Shantou Central Hospital were selected. 48 patients from March to October 2013 were named as Group A, and was processed by fast-track surgery concept; 49 patients from August 2012 to March 2013 were set as Group B and these patients were processed by routine nursing. The indexes such as post-operative drainage volume, post-operative evacuation time, time for removal of chest drainage tube, times of analgesic medication, post- operative hospitalization time, total hospitalization cost, post-operative complications and satisfaction of patients were compared. Results There were significant differences in post-operative evacuation time, time for removal of chest drainage tube, times of post-operative analgesic medication, post-operative hospitalization time and satisfaction of patients between group A and group B, but no significant difference was showed in post-operative drainage volume, total hospitalization cost and pos- t-operative complications. Conclusions Nursing intervention of the fast-track surgery can effectively promote patients＂ rapid recovery and improve the clinical outcomes and physician-patient relationship.
