摘要
目的探讨临床中亚低温血液净化治疗心脏瓣膜病术后低心排血量综合征的临床疗效。方法采取前瞻性研究方法,对2012年6月至2013年6月间76例心脏瓣膜病术后低心排血量综合征患者按照随机数字表法分为观察与对照两组,均38例。对照组给予常温血液净化治疗,观察组给予亚低温血液净化治疗,观察两组患者的临床治疗效果。结果治疗3 d后观察组与对照组APACHEⅡ评分、MODS评分、氧合指数、DO2/VO2、CI、PAWP较治疗前有不同程度的改善,且组间的比较差异有统计学意义(P<0.05);观察组留ICU时间、呼吸机辅助时间均低于对照组,差异有统计学意义(P<0.05);观察组感染率、心律失常率和死亡率均低于对照组(P<0.05),差异有统计学意义。结论临床中心脏瓣膜病术后低心排血量综合征患者应用亚低温血液净化治疗效果显著,有效的改善患者循环功能,减缓病情,缩短住院时间,值得临床中应用与推广。
ObjectiveTo evaluate the clinical efficacy of low blood purification therapy in clinical central valvular heart disease with postoperative low cardiac output syndrome.MethodsWith proactive research methods, from June 2012 to June 2013, 76 cases with postoperative low cardiac output syndrome in patients with heart valve disease according to random number table method were divided into observation and control two groups, all were 38 cases. The control group received normal temperature blood purification treatment, observation group were given mild hypothermia blood purification therapy, clinical treatment effect was observed in two groups.Results In 3 days after treatment, observation group and control group had different degrees of improvement than before treatment, in APACHE score, MODS score, oxygen index, DO2/VO2, CI, PAWP, the difference was statistically significant (P〈0.05); in observation group, the ICU time, mechanical ventilation time were lower than the control group, the difference was statistically significant (P〈0.05); in observation group, the infection, arrhythmia and mortality rates were lower than the control group, the difference was statistically significant (P〈0.05). Conclusion Clinical cardiac valvular disease in patients with low cardiac output syndrome after application of blood purification treatment effect is remarkable, and effectively improve the circulation function, patients with slow disease, shorten hospitalization time, is worth in the clinical application and promotion.
