摘要
目的了解2013年安徽省滁州地区细菌耐药性监测结果。方法采用纸片扩散法(K-B)对滁州地区3所医院所分离菌株进行药敏试验,按CLSI 2012年版标准判断结果;采用WHONET5.6进行耐药性分析。结果共收集临床分离菌2 927株,其中革兰阳性菌763株,占26.1%;革兰阴性菌2 164株,占73.9%。葡萄球菌属中耐甲氧西林金黄色葡萄球菌(MRSA)和耐甲氧西林凝固酶阴性葡萄球菌(MRCNS)分别占金黄色葡萄球菌和凝固酶阴性葡萄球菌的33.2%和85.9%。葡萄球菌属和肠球菌属中未见万古霉素和利奈唑胺耐药株。大肠埃希菌、克雷伯菌属(肺炎克雷伯菌和产酸克雷伯菌)和奇异变形杆菌产超广谱β内酰胺酶的检出率分别为56.9%、31.3%和22.8%。肠杆菌科细菌对美罗培南和亚胺培南较敏感。铜绿假单胞菌对美罗培南、亚胺培南、阿米卡星、哌拉西林-他唑巴坦和头孢他啶的耐药率均低于30%。不动杆菌属对美罗培南和亚胺培南的耐药率分别为58.1%和50.2%。结论细菌的耐药性日趋严重,对多重耐药和泛耐药菌株应引起高度重视。
Objective To investigate the antibiotic resistance in the clinical isolates collected in Chuzhou hospitals during 2013.Methods Antimicrobial susceptibility testing was carried out by Kirby-Bauer method.The data were analyzed by using WHONET5.6 software according to CLSI 2012 breakpoints.Results A total of 2 927 clinical isolates were collected during2013,of which gram positive organisms and gram negative organisms accounted for 26.1% and 73.9%,respectively.The prevalence of methicillin-resistant strains among S.aureus and coagulase-negative isolates were 33.2% and 85.9%,respectively.Staphylococcus,E.faecalis and E.faeciumisolates were 100% susceptible to vancomycin and linezolid.The prevalence of extended-spectrumβ-lactamases(ESBLs)positive strains was 56.9%,31.3% and 22.8%in E.coli,Klebsiella spp.(K.pneumoniae and K.oxytoca)and P.mirabilis,respectively.The Enterobacteriaceae strains were highly sensitive to carbapenems.The percentage of the P.aeruginosa isolates resistant to meropenem,imipenem,amikacin,piperacillintazobactam or ceftazidime was lower than 30%.The percentage of Acinetobacter spp.strains resistant to meropenem and imipenem was 58.1% and 50.2%.Conclusions The issue of antibiotic resistance is still very serious in 2013,especially the emerging multi-drug or pan-drug resistant strains,which is of great concern.
