摘要
通过对慢病管理模式总结与对比,讨论体检中心健康指导和慢病管理的模式设计。该模式设计要素:按照标准要求规划、设计体检中心职能;科学设置体检项目组合;出具个性化的健康保健计划和慢病管理策略;在医院、体检中心和社区之间建立一条医药卫生信息链。该模式设计旨在为体检中心提供科学、合理的健康指导及慢病管理的理论基础。
Through summarizing and comparison of chronic disease management mode, the scientific mode of health guidance and chronic disease management in medical examination center was discussed. Elements of the mode include the standard requires planning, the function of a medical center task, the medical science portfolio, the personalized health care plan and chronic disease management strategy, building a medical information chain among hospital, medical and community. The mode can provide scientific and reasonable theoretical basis for medical examination center.
