摘要
目的探讨应用血清降钙素原(PCT)和C-反应蛋白(CRP)水平评价美罗培南和头孢他啶对中枢神经系统感染疗效,为合理应用抗菌药物提供依据。方法对医院2011年3月-2013年1月收治的90例中枢神经系统革兰阴性杆菌感染患者,采取两种不同的抗菌药物治疗,A组48例患者采用美罗培南治疗,B组42例患者采用头孢他啶治疗,对两组疗效进行分析,同时比较患者血清降钙素原和C-反应蛋白水平的变化,并与30名健康者为对照组进行比较。结果两种抗菌药物对中枢神经系统革兰阴性杆菌感染治疗,A组总有效率为95.84%、B组为80.95%,差异有统计学意义(P<0.05);同时两组患者发病后血清降钙素原和C-反应蛋白水平要显著高于对照组,而经过相同时间治疗后,A组两种指标要显著优于B组,差异有统计学意义(P<0.05);血清降钙素原和C-反应蛋白水平对中枢系统感染诊断具有较高的灵敏度、特异度和准确度。结论中枢神经系统感染患者降钙素原和C-反应蛋白水平可以作为中枢系统感染程度的指标,同时不同抗菌药物对中枢神经系统感染疗效不同而影响降钙素原和C-反应蛋白水平。
OBJECTIVE To investigate the application the level of serum PCT and CRP to evaluate efficacy of meropenem and ceftazidime on the treatment of central nervous system infections so as to provide basis for reasonable use of antibiotics.METHODS The 90 patients with central nervous system infected by gram-negative bacteria in our hospital were collected from Mar.2011 to Jan.2013 and were treated with two different antibiotics.Group A with 48 patients,was treated with meropenem for therapy;group B with 42 patients,was adopted ceftazidime.The efficacy of the two groups were analyzed simultaneously,at the same time,the levels of serum procalcitonin and C-reactive protein were compared,then compared them with 30 normal volunteers,which served as the control group.RESULTS The two antibiotics were different on the effects to central nervous system infected by gram-negative bacteria.The total effective rate was 95.84%in the group A and 80.95%in the group B.The difference was significant(P〈0.05).Meanwhile,the serum procalcitonin and C-reactive protein levels of the two groups were significantly higher than the volunteer group,and after the same time treatment,the indicators in group A was significantly better than group B(P〈0.05).The Serum procalcitonin and C-reactive protein levels had high sensitivity,specificity and accuracy for the diagnosis of central nervous system infections.CONCLUSION The procalcitonin and C-reactive protein levels in patients with central nervous system infections can be used as indicators for central nervous system infections.The different curative effect of antibiotics on central nervous system infections impacts the level of procalcitonin and C-reactive protein.
