摘要
目的:观察中药坐浴及美皮康银离子辅料换药对肛周脓肿术后康复的影响。方法:将肛周脓肿术后患者76例随机分为治疗组40例和对照组36例。治疗组采用中药煎剂坐浴后美皮康银离子辅料伤口换药,对照组用1∶5 000高锰酸钾溶液坐浴后用呋喃西林纱条换药。比较2组疗效及术后第1、3、7天换药时疼痛情况和伤口渗液情况,住院时间。结果:治疗组治愈率为87.5%,对照组为63.9%,治疗组疗效优于对照组(P<0.05);治疗组平均住院时间为(7.50±1.01)天,较对照组(8.44±1.12)天短(P<0.05);治疗组术后第1、3天换药时伤口疼痛评分及渗液量均较对照组轻(P<0.05),术后第7天2组换药时伤口疼痛评分、渗液量比较无差异(P>0.05);换药过程中时间因素是缓解伤口换药疼痛及渗液量的独立影响因素(P<0.05),去除时间因素后两种方法比较差异有统计学意义(P<0.05)。结论:肛周脓肿术后采用中药坐浴及美皮康银离子辅料换药能提高临床疗效,更好地缓解恢复过程中伤口疼痛和伤口渗液等症状、缩短住院时间。
Objective： To investigate the effect of sitz bath with Chinese herbs assisted with Mepilex Ag excip-ient on patients undergoing the perianal abscess surgery. Methods： All 76 patients undergoing the perianal abscess surgery were randomized into the treatment group 40 cases and the control group 36 cases. Patients in the treatment group were performed sitz bath by Chinese herbs and applied Mepilex Ag excipient on the wound, while in the con-trol group 0.2‰potassium permanganate solution in the sitz bath and furacilin gauze on the wound were used to treat patients after the perianal abscess surgery. The curative effect of the two groups and the conditions of pain, wound exudates and hospitalization duration on the first, third and seventh day were compared. Results： The recov-ery rate in the treatment group were 87.5% and in the control group 63.9%, so that the therapeutic effect of the treat-ment group is superior to the control group （P〈0.05）; the average hospitalized duration was （7.50±1.01） day in the treatment group shorter than the control group （8.44±1.12） day （P〈0.05）; on the first and the third day the condition of pain and wound exudates in the treatment group were milder than that in the control group （P〈0.05）, and on the seventh day the condition in the two groups had no significant difference （P〉0.05）; in the process of dressing medicine time is the independent factor influencing the relief of pain and exudates, which was excluded in the com-parison, and the differences in treating patients by the two methods had statistical meaning （P〈0.05）. Conclusion： The therapy of Chinese herbs sitz bath integrated Mepilex Ag excipient for the recovery of patients undergoing the perianal abscess surgery can improve the clinical efficacy, greatly relieve the pain and wound exudates and shorten the hospitalization duration.
