摘要
目的探讨2型糖尿病患者糖尿病肾病的危险因素。方法选择2012年7月至2014年2月我院接诊的200例2型糖尿病患者进行研究。其中糖尿病肾病患者100例,非糖尿病肾病患者100例。分析比较两组患者的临床资料(年龄、病程)和生化指标(FPG、2h PG、TG、TC、BMI、SBP、DBP、UAER、INS、2h-INS、Hb A1c)以及把患者的各个因素与UAER进行多元逐步回归分析。结果观察组与对照组的收缩压为(19.21±1.23)和(16.97±1.14)k Pa,两组比较具有统计学意义(P<0.0001);观察组患者的病程、血糖、血压、三酰甘油、与胰岛素明显高于对照组,两组相比较具有统计学意义(P<0.05);多元回归分析的结果显示,病程、餐后血糖、空腹和空腹胰岛素、收缩压、舒张压、三酰甘油是糖尿病肾病的相关危险因素。结论病程、高血压、高胰岛素血症、高血糖、高血脂是糖尿病肾病的危险因素,应根据相关的危险因素对患者进行治疗。
ObjectiveTo evaluate the risk factors of diabetic nephropathy in type 2 diabetes.Methods 200 patients with type 2 diabetes from July 2012 to February 2014 in our hospital were researched. Among them, 100 cases of diabetic nephropathy, 100 cases of non-diabetic nephropathy. Two groups’ clinical data（age, duration） and biochemical indicators（FPG, 2hPG, TG, TC, BMI, SBP, DBP, UAER, INS, 2h-INS, HbA1c）were compared, various factors and UAERM was analyzed of ultiple stepwise regression.Results Systolic blood pressure in the observation group and the control group was（19.21±1.23）kPa and（16.97±1.14）kPa, there were significant differences compared to each other（P〈0.0001）; Duration, blood glucose, blood pressure, triglyceride, and insulin of observation group was higher than the control group, there was a significant difference（P〈0.05） . Multiple regression analysis showed that duration, postprandial glucose, fasting and fasting insulin, systolic blood pressure, diastolic blood pressure, triglycerides are risk factors associated with diabetic nephropathy.Conclusion The risk factor for kidney disease is duration of disease, hypertension, hyperinsulinemia, hyperglycemia, diabetes, high cholesterol. The clinical efficacy and the prognosis for Laparoscopic myomectomy. It is worthy of promotion in clinic. The patient should be treated according to the relevant risk factors.
