摘要
目的 探讨人类白细胞抗原(HLA)-DQA1基因拷贝数多态性与胃癌易感性的相关性,以及其与幽门螺杆菌(Hp)感染在胃癌发生中的交互作用.方法 利用微阵列-比较基因组杂交定位和AccuCopy多重基因拷贝数检测技术,对53例经病理检查确诊的胃癌患者和50例体检的正常人群的HLA-DQA1基因拷贝数多态性进行检测.利用Logistic回归模型分析HLA-DQA1基因拷贝数多态性与胃癌易感性的关系,以及与Hp感染的交互作用对胃癌易感性的影响.结果 HLA-DQA1基因拷贝数多态性分布和Hp感染阳性率在两组对象中不同,差异有统计学意义(P <0.05);HLA-DQA1基因拷贝数>2型人群胃癌易感性显著增加[比值比(OR)=1.79,95%可信区间(CI):1.13 ~ 3.22,P<0.05];HLA-DQA1基因拷贝数>2且Hp感染阳性人群胃癌易感性显著增加(OR=3.31,95%CI:1.69~7.88,P <0.05).结论 HLA-DQA1基因拷贝数多态性与胃癌易感性密切相关,而与Hp感染无明显相关,但两者在胃癌发生进展过程中共同发挥交互作用.
Objective To investigate the correlation of human leukocyte antigen (HLA)-DQA1 gene copy number polymorphisms and susceptibility to gastric cancer,and to explore their interaction with Helicobacterpylori (Hp) infection in gastric carcinogenesis.Methods HLA-DQA1 gene copy number in 53 cases of pathologically diagnosed gastric cancer and 50 cases of normal healthy population was detected by using microarray-comparative hybridization and AccuCopy multiple gene copy number detection technology.Logistic regression analysis was used to evaluate the impact of this polymorphism on the risk of susceptibility to gastric cancer and the gene-Hp infection interaction.Results There was statistically significant difference in the HLA-DQA1 gene copy number polymorphisms and positive rate of Hp infection between the two groups (P 〈 0.05).The susceptibility of population to gastric cancer with HLA-DQA1 gene copy number 〉2-type was significantly increased [odds ratio (OR) =1.79,95% confidence interval (CI):1.13-3.22,P 〈0.05).The susceptibility of population to gastric cancer with HLA-DQA1 gene copy number 〉 2-type and Hp positive infection was significantly increased (OR =3.31,95% CI:1.69-7.88,P 〈 0.05).Conclusion HLA-DQA1 gene copy number polymorphisms were closely related with susceptibility to gastric cancer,while were not with Hp infection,but there was a multiplicative interaction between these polymorphisms and Hp infection in the process of gastric cancer.
