摘要
目的 总结经尿道同期治疗前列腺增生并发膀胱结石的临床经验.方法 分析采用经尿道等离子前列腺切除联合气压弹道碎石术同期治疗前列腺增生并发膀胱结石33例患者的临床疗效与不良反应发生情况.结果 所有病例均一次治疗成功,2例术后出现尿道外口狭窄,无严重出血、电切综合征发生,手术时间57～186min,平均（93±24）min;国际前列腺症状评分由术前（26.3±5.4）分降为术后1个月的（7.6±3.3）分（P＜0.01）,手术前后膀胱残余尿量分别为（176.5±62.7）ml和（15.4±6.5）ml（P＜0.01）,结石清除率100％;术后随访7～32个月,无膀胱结石复发.结论 经尿道等离子前列腺切除联合气压弹道碎石是同期治疗前列腺增生并膀胱结石的有效方法;依据术中情况,先行经尿道前列腺切除,后行气压弹道膀胱碎石可提高手术效率.
Objective To summarize the method and efficacy of combined Plasma Kinetic transurethral resection of the prostata（PKRP）and transurethralpneumatic ballistic lithotripsy（TULT）for BPH in patients with vesical calculi.Methods 33 patients with BPH with multiple vesical calculi undergone PKRP,followed by lithotripsy（TULT）.The average stones diameter of calculi was15.4mm（range 1.0-3.5cm）.The indexes such as operative time,stone free rate,international prostate symptom score（IPSS）,and postvoid residual urine volume（PVR）were monitored and analyzed at the preoperative and postoperative.Results All 33 cases were successfully operated in a single session.Satisfactory voiding was achieved in all except two patient developed urethral meatus stricture.None of them had severe hemorrhage and transurethral resection syndrome（TURS）.The mean performed time was（93±24）minutes.The average weight of the prostate was52 grams.IPSS decreased from（26.3±5.4）to（7.6±3.3）at 1month postoperative（P〈0.01）and PVR dropped from（176.5±62.7）ml to（15.4±6.5）ml（P〈0.01）.The stone free rate was 100%.Conclusion According to the intra-operative conditions,the procedure that PKRP performed firstly,followed by TULT and removal of the calculi,can improve obviously the operative efficiency in a single session.
