摘要
目的探讨经颈超声测量鼻咽癌原发肿瘤体积与颈淋巴结转移率及大小的关系.资料与方法91例鼻咽癌患者首次放疗前行二维超声检查,测量肿瘤及淋巴结大小,比较不同体积肿瘤的颈淋巴结转移率及肿瘤大小与淋巴结大小的关系.结果91例鼻咽癌患者中,颈淋巴结转移61例,原发肿瘤体积≤10cm3、11～30cm3、31～50cm3和≥51cm3者的颈淋巴结转移率分别为44.4％、78.1％、82.4％、66.7％,各组间比较差异有统计学意义（x2=9.82,P＜0.05）;其中颈淋巴结最大径≥2cm者在不同体积组分别占16.7％、80.0％、78.6％和50.0％,颈淋巴结最大径＜2cm者分别占83.3％、20.0％、21.4％和50.0％,两者在不同体积组间转移率差异有统计学意义（x2=16.20,P＜0.01）.结论鼻咽癌原发肿瘤体积与颈淋巴结转移率及大小密切相关,经颈超声测量鼻咽癌原发肿瘤体积简便易行,可以为研究鼻咽癌转移和预后规律提供重要线索.
Purpose To investigate the relationship between primary tumor volume of nasopharyngeal carcinoma measured by ultrasound and the size of metastatic cervical lymph nodes. Material and Methods Ninety-one patients with nasopharyngeal carcinoma underwent two-dimentional ultrasound exam before initial radiation therapy to measure the size of primary tumor and lymph nodes. The relationship between the tumor volume with the incidence and size of metastatic cervical lymph nodes were analyzed. Results Sixty-one patients were found to have lymph nodes metastasis. The incidence was 44.4%, 78.1%, 82.4% and 66.7% tbr primary tumor volume of≤ 10 cm3, 11-30 cm3, 31-50 cms and 〉 51 cms 0（2=9.82, P〈0.05）. There were 16.7%, 80.0%, 78.6% and 50.0% lymph nodes ≥ 2 cm in diameter, and 83.3%, 20.0%, 21.4% and 50.0% lymph nodes〈2 cm respectively, these differences were statistically significant （χ^2=16.20, P〈0.01）. Conclusion In nasopharyngeal carcinoma, primary tumor volume is closely related to cervical lymph node metastasis. Ultrasound measurement is simple and feasible. It provides important information for the investigating metastasis and prognosis.
