摘要
目的探讨脑小血管病(SVD)患者的血浆同型半胱氨酸(Hcy)水平及其与认知功能障碍的关系。方法选择SVD患者69例和同期健康体检者(对照组)40例作为研究对象,对受试者进行蒙特利尔评估量表(MoCA)、日常生活活动能力(ADL)神经心理学量表检查,将SVD患者依据认知功能水平分为认知功能正常(NCI,28例)组与认知功能障碍非痴呆(VCIND,41例)组。抽取空腹静脉血,采用化学发光法测定研究对象血浆Hcy水平,分析Hcy水平与MoCA评分的相关性。结果与对照组相比较,NCI组和VCIND组的Hcy水平显著升高(P<0.05),且VCIND组较NCI组更高(P<0.05)。Spearson相关分析示,SVD患者Hcy水平与MoCA总分及视空间与执行功能、注意与计算力、抽象力亚项评分呈负显著相关性(r=-0.484,P=0.000;r=-0.370,P=0.002;r=-0.392,P=0.001;r=-0.289,P=0.016)。结论 Hcy水平升高是脑小血管病性认知功能损害的独立危险因素,血浆Hcy浓度的高低与SVD患者认知功能损害的程度相关,早期检测Hcy具有重要意义。
Objective To explore the plasma level of homocysteine(Hcy) and its correlation with cognitive impairment in patients with cerebral small vessel disease (SVD). Methods In this study, 69 SVD patients and 40 normal health subjects( control group) were observed, Montreal cognitive assessment(MoCA) scores and the activities of daily living(ADL) were assessed. The SVD patients were divided into normal cognition group( NCI group,n = 28 ) and cognitive impairment without dementia group( VCIND group, n = 41 ). In all the subjects, fating venous blood were drawn to determine Hcy level by chemiluminescence method,Correlation between plasma Hcy level and MoCA scores was analyzed. Results In comparison to normal individuals, plasma levels of Hcy were higher in the NCI and VCIND patients (P 〈 0.05 ), and Hcy level in VCIND group was significantly higher than NCI group( P 〈0.05 ). Spearson correlation analysis showed that the plasma level of Hcy were negative correlation with MoCA total score and MoCA subscore with visual-spatial and executive function, attention and calculation, abstracting ability in all the SVD patients ( r = - 0. 484, P = 0. 000 ; r =- 0. 370, P = 0.002;r = -0.392,P=0.001;r = -0.289,P=0.016). Conclusion Elevated Hcy level was an independent risk fac- tor for cognitive function impairment, the concentration of plasma Hcy was correlated with the degree of cognitive impairment in SVD patients,it was of great significance to detect Hcy early.
