哮喘儿童诱导痰中炎性细胞及细胞因子的检测 被引量:5

摘要 目的了解炎性细胞及细胞因子在儿童哮喘中的作用及其机制。方法应用实验研究策略,选择重症哮喘急性发作期组及其取得临床缓解期组30例,正常对照组30例。进行肺功能检测,采用诱导痰检查法对痰炎性细胞进行分类计数,采用酶联免疫吸附法测痰IL-8、IL-17浓度。三组间均数比较,应用单因素方差分析,任意两组间比较用SNK法;并进行直线相关性分析。结果经三组间单因素方差分析比较,哮喘急性发作期组诱导痰中性粒细胞含量、IL-8和IL-17含量均高于临床缓解期组及正常对照组,差异具有统计学意义(F值分别为172.16、135.69和24.28,均P<0.05)。临床缓解期组诱导痰的中性粒细胞含量IL-8和IL-17含量高于正常对照组,差异具有统计学意义(F值分别为67.80、54.64和17.36,均P<0.05);急性发作期及临床缓解期IL-8、IL-17均与中性粒细胞成正相关(均r>0,P<0.05)。结论中性粒细胞、IL-8、IL-17均参与了哮喘的部分发病机制,对于哮喘病情变化、诊断和治疗方面的研究提供了进一步探索的基础。 Objective To understand the effect and mechanism of inflammatory cells and cytokine in childhood asthma. Methods Experimental research strategy was adopted to select 30 cases at acute stage and at clinical remission stage, and 30 normal controls as well. Their pulmonary function was tested and sputum induction method was used to calculate sputum inflammatory cells. The coneerttrations of sputum IL-8 and IL-17 were measured by enzyme-linked imnlunosorbent assay. Single factor variance analysis was conducted to compare mean values of three groups, SNK method was used to make pair comparison, and linear correlation analysis was carried out. Results Single factor variance analysis showed that the contents of neutrophils, IL-8 and IL-17 in induced sputum at acute stage group were higher than clinical remission group and control group, and the differences were significant (F value was 172.16, 135.69 and 24.28, respectively, all P 〈 0.05). In clinical remission stage group the contents of neutrophils, IL-8 and IL-17 in induced sputum were significantly higher than the normal control group ( F value was 67.80, 54.64 and 17.36, respectively, all P 〈 0.05). At acute stage and clinical remission stage the contents of IL-8 and IL-17 were positively correlated with neutrophils ( r 〉 0, both P 〈 0.05 ). Conclusion All of neutrophils, IL-8 and IL-17 involve in the pathogenesis of asthma, which provides basis for further exploration on diagnosis and treatment.
