摘要
目的通过观察关节腔注射褪黑素治疗后骨性关节炎大鼠关节软骨中碱性成纤维细胞生长因子(bFGF)和白细胞介素-1β(IL-1β)的表达,探讨褪黑素对关节软骨损伤的修复作用。方法将大鼠随机分为4组,每组10只。A组为正常对照组,B组为骨性关节炎组,C组为骨性关节炎+去松果体模型组,D组在C组基础上给予关节腔注射褪黑素治疗(大鼠成功建模1周后,每周4次给予左膝关节腔注射浓度为20g/L褪黑素溶液0.2mL,共4周)。最后1次注射褪黑素后,采集A、B、C组大鼠心尖处血液,应用酶联免疫吸附法检测血清褪黑素水平。然后处死全部大鼠,取左膝股骨髁软骨行大体观察、组织学及免疫组织化学染色观察。结果 A组软骨表面光滑有弹性,边缘规整,软骨细胞排列整齐;B组软骨表面粗糙无光泽,存在软骨软化及软骨下骨外露现象,软骨细胞排列及结构紊乱;C组较B组严重;D组软骨软化及软骨剥脱现象较B、C组减轻,软骨下骨外露减少,软骨细胞排列及结构较为整齐。D组大鼠关节软骨中IL-1β的表达显著低于C组和B组,bFGF的表达显著高于C组和B组(F=225.73、285.25,P<0.05)。结论关节腔注射褪黑素能够延缓大鼠骨性关节炎的进一步发展,其作用机制可能与上调软骨生长因子bFGF及下调炎性因子IL-1β表达有关。
Objective To investigate the effect of intra articular injection of melatonin on the articular cartilage injury in rats with osteoarthritis by observing the expressions of bFGF and IL-1β. Methods A total of 40 rats were evenly randomized to four groups: group A, normal control group; group B, osteoarthritis group; group C, osteoarthritis plus pinealectomy model group; group D, intra-articular injection of melatonin on the basis of group C. One week after the model was successfully created, the rats in group D were injected with 0.2 mL 20 g/L melatonin solution in left knuckle articular cavity four times a week for four weeks. After the last melatonin injection,the blood of rats in groups A, B and C was taken from the apex of heart for serum melatonin detection by ELASA. The rats in all groups were then sacrificed, the tissue from left cartilage of femoral condyle was collected for gross observation, histology and immunohistochemical staining. Results The findings in each group were as follows: group A-the surface of articular cartilage was smooth and elastic with regular edge, the ehondrocytes lined up in order; group Bthe surface of articular cartilage was rough and shineless; group C-the condition was more serious than in group B; group D-chondro malacia and cartilage exfoliation were lightened versus that seen in groups B and C, subehondral bone reveal decreased, alinement and structure of cartilage cells were regular. The expression of IL-β in articular cartilage was significantly lower than groups C and B (F=225.73,P〈0.05), and that of bFGF was higher (F=285.25,P〈0.05). Conclusion Intra articular injection of melatonin can delay further progress of osteoarthritis in rats, the mechanism is probably to be related with up-regulation of cartilage growth factor bFGF and down-regulation of inflammatory factor IL-1β.
