摘要
目的研究进展期胃癌侵犯胃结肠韧带(GCL)的多层螺旋CT表现并探讨其解剖学基础。方法回顾性分析经手术病理证实的58例进展期胃癌的CT和临床资料,着重分析GCL受累CT表现并与其手术病理结果相对照。结果进展期胃癌侵犯GCL的CT表现为GCL内的实性肿块形成、与癌肿相续的结节影和条索影、粟粒性结节影、脂肪密度增高和淋巴结增大等。结论进展期胃癌侵犯GCL具有一定的CT表现特征,多层螺旋CT在术前评价GCL受侵方面具有重要价值。
Objective To evaluate multiple-slice spiral CT （MSCT） features of the involvement of gastrocolic ligament （GCL） in advanced gastric carcinomas, and to discuss its anatomic bases. Methods A total of 58 patients with pathologically-proved advanced gastric carcinomas complicated by involvement of GCL were enrolled in this study. The clinical data and CT scanning materials were retrospectively analyzed, with a focus on the CT manifestations of the involvement of GCL and the correlation between CT findings and the pathologic results. Results In advanced gastric carcinomas the involvement of GCL on CT scans was characterized by the formation of a solid mass within GCL together with nodular, cord-like and/or miliary shadows, increased density of fat, lymphadenopathy, etc. Conclusion The involvement of GCL in advanced gastric carcinomas has certain CT characteristics. MSCT scanning carries certain high value in evaluating the involvement of GCL due to gastric carcinoma.
