地佐辛复合咪达唑仑在纤维支气管镜引导清醒气管插管中的临床观察 被引量:2

摘要 目的观察地佐辛复合咪达唑仑在纤维支气管镜引导清醒气管插管中的镇痛、镇静效果以及对循环和呼吸功能的影响。方法困难气管插管患40例(ASAⅠ-Ⅱ级),分为地佐辛+咪达唑仑(D组,n=20)和芬太尼+咪达唑仑(F组,n=20),所有患者均以1%丁卡因表面麻醉后纤维支气管镜引导经口清醒气管插管,于用药前(T0)、用药后5 min(T1)和气管插管后即刻(T2),记录循环功能和呼吸参数的变化、镇静Ramsay评分及可能出现的不良反应等。结果所有病例均顺利完成气管内插管。两组镇静程度差异无统计学意义(P〉0.05)。与T0比较,两组患者T1时HR和MAP均显署下降(P〈0.05),但F组有8例(40%)注药5min后出现以呼吸频率减慢为主的呼吸抑制。结论在表面麻醉下,采用0.1mg/k地佐辛复合咪达唑仑静脉注射能产生有效的镇静和镇痛效应,适合应用于纤维支气管镜引导下清醒气管插管。
