摘要
目的探讨超声内镜联合内镜下黏膜切除术(EMR)与黏膜剥离术(ESD)治疗上消化道黏膜下肿瘤(SMTs)的临床价值。方法对2010年2月至2011年2月进行EMR与ESD治疗的50例上消化道SMTs患者的病例资料进行回顾性分析,总结其治疗效果。结果 50例上消化道SMTs中,位于食管10例,胃40例。术前内镜下超声小探头诊断间质瘤15例、异位胰腺17例、脂肪瘤18例。其中30例行EMR,20例行ESD。病灶均完整切除,未发生出血、穿孔等并发症,术后病理组织学均符合超声内镜检查结果。患者术后均随访两年,无一例复发。结论经内镜治疗上消化道黏膜下肿瘤安全、有效。
Objective To evaluate the efficacy and safety of upper gastrointestinal submucosal tumors with endoscopic management.Methods The analysis method with endoscopic management in 50 cases of upper gastrointestinal submucosal tumors in our hospital were analyzed retrospectively in patients and summarize the treatment effect. Results Of 50 upper gastrointestinal submucosal tumors,10 located in esophagus,40 in stomach. By endoscopic ultrasonography,30 were managed by endoscopic mucosal resection,20 by endoscopic submucosal dissectios. 50 patients there were no massive hemorrhage,perforation and other complications. 13 Lesions were histopathologically diagnosed as leiomyoma,2 as gastrointestinal mesenchymal tumor,17 etopic pancreas,18 gastric lipomas. No case relapsed in 2 years followed-up.Conclusion Endoscopic mucosal resection and Endoscopic submucosal dissectios is a safe and effective treatment for upper gastrointestinal submucosal tumors.
