保留后韧带结构的椎管成形术治疗腰椎管狭窄症 被引量:3

摘要 目的 探讨使椎板开门椎管扩大成形术更完善的方法。方法 设计保留后韧带结构的椎板开门椎管扩大成形术治疗椎管狭窄症 10 7例。结果 术后矢状径平均为 1 7cm ,平均扩大 0 75cm ,截面积平均 1 86cm2 ,平均增加 0 91cm2 。随访 87例 ,时间为术后 5~ 8年。疗效评定 :优 6 4例 ,良 2 3例。结论 后韧带结构为坚韧有弹性的骨 韧带条 ,在维护脊柱稳定的过程中 ,有无法替代的价值。术式既有后路手术直视减压的优点 ,又有效地扩大椎管矢状径 ,增加截面积 ,术后脊柱较稳定。 Objective To describe a method for the improvement of opening door laminoplasty in treating spinal canal stenosis Methods 107 cases of spinal canal stenosis were treated with opening door laminoplasty which preserved the intact posterior complex structures.Results The sagittal diameter of spinal canal was 1 7cm and was enlarged by 0 75cm on the average.The section area of spinal canal was 1 86cm 2 and was enlarged by 0 91cm 2 on the average.87 cases were followed up and the follow up period ranged from 5 to 8 years.64 cases achieved excellent and 23 cases good results.Conclusion The posterior complex structures are the bone and ligament strips which are strong and elastic.Their function could not be replaced on the protection of the stability of spine.This operative procedure has the advantage of laminectomy and decompression and is also able to enlarge the sagittal diameter effectively and increases section area.The lumbar spine is more stable than merely carrying out laminectomy.
