摘要
目的 比较自体角膜缘上皮移植术和传统的单纯翼状胬肉切除术对初发性翼状胬肉的疗效。方法 将 2 0 8例 (2 2 9只眼 )初发性翼状胬肉患者随机分成A、B组 :A组 10 6例 (12 4只眼 )行翼状胬肉切除联合自体角膜缘上皮移植术 ;B组 10 2例 (10 5只眼 )行单纯翼状胬肉切除术。以上皮愈合稳定、角膜恢复正常光滑透明、无翼状胬肉样组织生长为治愈 ,否则视为复发。术后随访 18～ 2 8个月 ,平均 (2 2 4± 4 9)个月。比较观察两组患者术后翼状胬肉复发情况、视力及角膜散光度数。结果A组 10例 (9 4 % )患者失访 ,4例患者 (5只眼 )翼状胬肉复发 ,复发率为 4 5 % ;B组 8例 (7 8% )患者失访 ,4 0例患者 (41只眼 )翼状胬肉复发 ,复发率为 4 2 7% ,两组比较 ,差异有非常显著意义(P <0 0 0 1) ;手术前、后视力及角膜散光度数比较 ,差异有显著意义 (P值均 <0 0 5 ) ,两组患者术后视力及角膜散光度数比较 ,差异无显著意义 (P值均 >0 0 5 )。结论 自体角膜缘上皮移植术可有效治疗翼状胬肉 ,防止翼状胬肉引起角膜散光 。
Objective To observe the therapeutic effects of limbal epithelial autograft transplantation and pterygium excision in the treatment of pterygium Methods A prospective randomized paired eye trial was studied There were 208 patients (229 eyes) with initial pterygium, and they were allocated to two groups: excision of pterygium with limbal epithelial autograft transplantation surgery (A group, 106 cases and 124 eyes) and simple pterygium excision (B group, 102 cases and 105 eyes) The criteria for recovery were corneal transparency with stable epithelial healing and no abnormal proliferation of pterygium like tissue The post operative follow up periods ranged from 18～28 (22 4±4 9) months Results Some of the patients lost follow up In the eyes followed up, 5 of 112 eyes (4 5%) in A group and 41 of 96 eyes (42 7%) in B group were recurred, the difference being very significant ( P <0 001) Conclusion To provide a new stem cell source, limbal epithelial autograft transplantation, for an injured limbus is a reasonable therapeutic method for the treatment of pterygium
