全身炎症反应综合征患者致炎因子与抗炎因子的变化及相关性研究 被引量:44

Changes of inflammatory and anti-inflammatory cytokines and their correlation in patients with systemic inflammatory response syndrome

摘要 目的 观察全身炎症反应综合征 (SIRS)患者血浆中致炎因子肿瘤坏死因子 -α(TNF -α)、白介素 - 6 (IL - 6 )和抗炎因子转化细胞生长因子 - β(TGF - β)、白介素 - 10 (IL - 10 )的变化规律及其相关性。 方法 符合SIRS诊断 4 8例 ,分别在入院后第 1、8小时和第 2、3、4天清晨空腹抽肘静脉血 3mL ,测定TNF -α、IL - 6、TGF - β和IL - 10 ,并设对照组。 结果 TNF-α和IL - 6各监测点均比对照组显著升高 (P <0 0 5 ,P <0 0 1) ,TGF - β和IL - 10入院后第 1小时与对照组比较无显著差异 ,第 8小时以后各监测点均比对照组显著升高 (P <0 0 5 ,P <0 0 1) ,TNF -α和IL - 6峰值出现早于TGF - β和IL - 10 ,致炎因子与抗炎因子具有显著的相关性。结论 拮抗炎症因子的同时提高机体免疫功能 ,达到致炎因子与抗炎因子的平衡是治疗SIRS的新思路。 Objective To investigate the changes in inflammatory cytokines[tumor necrosis factor-α(TNF-α),interleukin-6(IL-6)]and anti-inflammatory cytokines[transforming growth factor-α(TGF-β),interleukin-10(IL-10)〕in plasma and their correlation in patients with systemic inflammatory response syndrome(SIRS).[WT5'HZ]Methods 48 patients diagnosed as SIRS were in the trial group while health people were in the control one.3-ml blood was SIRS were in the trial group while health people were in the control one.3-mL blood was drown form elbow vein in each patient respectively at 1st and 8th hours,as well as on the 2nd, 3rd and 4th day's mornings after their admissons.Plasma TNF-α,IL-6,TGF-β and IL-10 were detected.Results TNF-α and IL-6 were higher in the trial group during observation periods then in the control group(P<0 05,P<0 01).TGF-β and IL-10 at 1st hour were not obviously different between two groups,but they were higher from the 8th hour to 4th day in the trial group than in the control group (P<0 05,P<0 01) The peak values of TNF-α and IL-6 appeared earlier than ones of TGF-β and IL-10.There was a significant correlation between inflammatory and anti-inflammatory cytokines.Conclusion It was a new thinking to treat SIRS that the inflammatory cytokine was resisted and immune function was improved at the same time to reach tne balance between inflammatory and anti-inflammatory cytokines.
