摘要
目的 分析支气管结核的CT及支气管镜表现 ,探讨支气管结核的影像特征及其病理基础。方法 收集痰菌、病理及临床证实的支气管结核 5 0例 ,对照分析CT及支气管镜表现及CT的动态变化。结果 支气管结核CT表现有以下特点 :①病变范围广泛 ;②多数病例有肺内播散 ;③支气管狭窄、阻塞不伴有肺门肿块 ;④管壁内可见钙化。对照分析显示 :管腔狭窄由黏膜充血、水肿及溃疡引起 ,阻塞由肉芽肿和陈旧斑痕造成。动态观察治疗后多数病例肺内播散病灶吸收。结论 CT是诊断及鉴别支气管结核的有效手段 ,也可评价病变的动态变化。
Objective To analysis the manifestation of CT and bronchoscopy of bronchial tuberculosis, to evaluate CT features and the base of pathology. Methods Fifty patients of bronchial tuberculosis were proved by mycobacterium tuberculosis in sputum, pathology and clinic. The dynamic CT manifestation in the patients was analysed, and the features of CT and bronchoscopy were compared too. Results CT features of bronchial tuberculosis were as follow:①long range of lesion in bronchus, ②disseminating focus in the pulmonary of many patients, ③stenosis and obstruction of bronchus without mass in hilar, ④calcification in the bronchial wall. The comparing study of CT and bronchoscopy indicated: stenosis caused by the edema, congestion and ulcer of the mucosa and obstruction due to the granuloma and old scar. The dynamic observation demonstrated most disseminated focus disappear after therapy. Conclusion CT is the valuable method in diagnosis and differentiation of bronchial tuberculosis, also useful in evaluating the dynamic change.
