肺爆震伤治疗的研究进展 被引量:6

摘要 爆炸伤无论是在军事方面,还是在工业生产或民用方面都是一个日益严重的问题。爆炸产生强大的冲击波超压,对机体造成的损伤称为原发冲击伤或爆震伤。肺组织容易压缩,同时肺组织存在气液界面,因此更容易损伤,压缩或剪切力导致肺组织的破坏,并继发出血和水肿。肺爆震伤常缺乏外观可见的胸部伤口,早期症状和体征不一定很明显,其特有的肺部病理生理特点,给常规的液体复苏和机械通气带来挑战,导致救治困难,病死率高。本文将对肺爆震伤的发病机制、诊断、治疗及预后进行阐述。 Explosive injuries are a growing problem both in military, industrial and civilian areas.Primary blast injury result from the effects of the blast overpressure of an explosion.Because of the compressibility and their gas-liquid interface, the lungs are particularly vulnerable to this form of injuries.Compression or shear force leads to the destruction of lung tissue and secondary hemorrhage and edema.Lung blast injuries often lack visible chest wounds, and early symptoms and signs are not always obvious.Its unique pathophysiological characteristics bring challenges to conventional fluid resuscitation and mechanical ventilation, leading to treatment difficulties and high mortality.This article summarizes the pathogenesis, diagnosis, treatment and prognosis of lung blast injury.
