摘要
目的探索脉冲振荡法(IOS)检测支气管哮喘和慢性阻塞性肺疾病(COPD)呼吸阻抗与肺功能的关系。方法连续纳入2017年1—9月到我院就诊的患者,选取40例COPD的患者,设为A组;同期选取40名支气管哮喘患者,设为B组;选取40例健康体检者,设为C组。分别测定每个实验对象的肺功能指标和呼吸抗阻指标,分析两种疾病的呼吸抗阻和肺功能的相关性和COPD病情进展过程中呼吸抗阻的变化。结果 A组随着病情进展,R5、R5-20、Zrs、Rc、Rp显著增加,R20无明显变化。肺功能对比:A组和B组的FEV_1、FEV_1/FVC、FEV_1%pred明显高于C组。呼吸抗阻对比:A组的Zrs、R5高于C组,B组的R20高于C组,A组和B组的R5-20、Rp均高于C组。结论 COPD和支气管哮喘均可引起气道阻力增加,但哮喘以中心气道阻力增加为主,慢阻肺以外周气道阻力增加为主。肺功能检查是两种疾病主要诊断方法,IOS检测呼吸抗阻能较好地反映气道阻力及肺顺应性,进一步判断阻塞部位。
Objective To detect the respiratory impedance of patients with bronchial asthma and COPD by using IOS and explore the relationship between respiratory impedance and lung function.Methods Continuously enrolled cases those who went to our hospital for physical examination during January 2017 to September 2017.A total of 40 patients with chronic obstructive pulmonary diseases( COPD) were enrolled in the group A,40 patients with bronchial asthma were selected as group B,and 40 healthy subjects were selected as group C.The indexes of lung function and respiratory resistance index were measured in each test object.The correlation of respiratory resistance and pulmonary function and the changes of respiratory resistance during the progression of COPD were analyzed.Results In group A,with the progression of the disease,R5,R5-20,Zrs,Rc,and Rp increased significantly,but R20 had no obvious changes.Pulmonary function comparison: FEV1,FEV1/FVC and FEV1%pred in group A and group B were significantly higher than those in group C.The contrast of respiratory resistance: Zrs and R5 in group A were higher than that in group C,and R20 in group B was higher than that in group C,R5-20 and Rp in group A and B were higher than those in group C.Conclusions Both COPD and bronchial asthma could cause an increase in airway resistance,but asthma is mainly due to increased central airway resistance,and peripheral airway resistance increases mainly in chronic obstructive pulmonary disease.Pulmonary function test is the main diagnostic method for the two diseases.IOS detection of respiratory resistance can reflect airway resistance and lung compliance well,and further determine the obstruction site.
