摘要
目的探讨血清胆碱酯酶(ChE)与过氧化脂质(LPO)对重度脓毒症患者生存状况的影响。方法选取2014年2月-2017年2月医院收治的脓毒症患者78例为研究对象。根据国际脓毒症会议中有关脓毒症的诊断标准,分为严重脓毒症组24例和非严重脓毒症组54例。比较两组患者ChE和LPO指标水平。分析ChE和LPO指标单独与联合检测对严重脓毒症患者预后的诊断价值。结果严重脓毒症组ChE为(3120.56±652.35)U/L低于非严重脓毒症组的(4563.59±1201.55)U/L,LPO为(13.15±2.53)nmol/ml高于非严重脓毒症组的(10.02±2.16)nmol/ml,两组比较差异有统计学意义(P<0.05);严重脓毒症组中,死亡16例,存活8例。死亡组ChE水平低于存活组,LPO水平高于存活组,差异均有统计学意义(P<0.05);ChE和LPO指标联合检测诊断严重脓毒症患者预后的灵敏度和特异度分别为0.815和0.917,均优于二者单独预测参数。结论联合检测血清ChE与LPO有助于评估严重脓毒症患者的预后。
OBJECTIVE To explore the influence of serum cholinesterase(ChE)and lipid peroxidase(LPO)on survival of patients with severe sepsis.METHODS A total of 78 patients with sepsis who were treated in hospitals from Feb 2014 to Feb 2017 were recruited as the study objects and divided into the severe sepsis group with 24 cases and the very severe sepsis group with 54 cases according to the Criteria for diagnosis of sepsis by international conference for sepsis.The levels of chE and LPO were compared between the two groups of patients.The values of single detection and joint detection of ChE and LPO in prediction of prognosis of the patients with severe sepsis were observed.RESULTS The ChE level of the severe sepsis group was(3102.56±652.35)U/L,significantly lower than(4563.59±1201.55)U/L of the very severe sepsis group;the LPO level of the severe sepsis group was(13.15±2.53)nmol/ml,significantly higher than(10.02±2.16)nmol/ml of the very severe sepsis group(P<0.05).In the severe sepsis group,16 patients died,and 8 patients survived;the ChE level of the death group was lower than that of the survival group,the LPO level of the death group was significantly higher than that of the survival group(P<0.05).The sensitivity and specificity of the joint detection of ChE and LPO were respectively0.815 and 0.917 in the diagnosis of severe sepsis,higher than those of the single detection of ChE and LPO.CONCLUSION The joint detection of serum ChE and LPO may facilitate the prediction of prognosis of the patients with severe sepsis.
