摘要
目的研究不同透析方式对残余肾功能及预后的影响。方法选择该院2016年1月‐2018年1月纳入的200例终末期肾病患者,按照随机数表法分为两组各100例,其中研究组采取腹膜透析,对照组采取血液透析,比较两组治疗结果及患者残余肾功能及预后的影响。结果治疗前两组的残余肾功能、血肌酐、尿素氮、血红蛋白、白蛋白、总胆固醇及三酰甘油水平差异无统计学意义(P>0.05),治疗后研究组残余肾功能高于对照组,但其他指标水平均低于对照组(P <0.05);治疗前两组的躯体活动、饮食行为、社会心理行为及治疗行为评分差异无统计学意义(P>0.05),治疗后研究组各生存质量评分均高于对照组(P <0.05);研究组并发症发生率7.00%,低于对照组的19.00%(P <0.05)。结论血液透析与腹膜透析均在终末期肾病患者中具有一定价值,其中腹膜透析治疗效果更好,对患者残余肾功能具有保护作用,同时改善生存质量,促进病情快速稳定。
【Objective】To study the impact of different dialysis modes on the residual renal function and patient’s prognosis. 【Methods】Two hundred end-stage renal disease patients treated from January 2016 to January 2018 in our hospital were selected. Patients were grouped by the random number table, 100 cases in each group. The study group took the peritoneal dialysis;the control group took the hemodialysis. The therapeutic effect, residual renal function and patient’s prognosis were compared. 【Results】Before treatment, the index of residual renal function, serum creatinine, blood urea nitrogen, hemoglobin, albumin, total cholesterol and triglyceride between the two groups were not significantly different(P >0.05);after treatment, the residual renal function in the study group was increased while the other indexes were reduced, with significant differences between groups(P <0.05);before treatment, the scores of physical function, dietary behavior, social and psychological behavior and therapeutic behavior between the two groups were not significantly different(P >0.05);after treatment, the quality of life scores in the study group were higher than the control group(P <0.05);the incidence rate of complications in the study group(7.00%) was significantly lower than the control group(19.00%)(P <0.05). 【Conclusion】Both hemodialysis and peritoneal dialysis has its application value in endstage renal disease patients. However, the peritoneal dialysis can better increase the therapeutic effect, protect the residual renal function, improve the quality of life and stabilize the illness conditions.
